口腔颊黏膜对比舌黏膜前尿道成形术有效性和安全性的系统评价 被引量:6

Buccal versus lingual mucosa graft in anterior urethroplasty:a systematic review and meta-analysis

摘要 目的:系统评价口腔颊黏膜对比舌黏膜尿道成形术的有效性和安全性。方法:计算机检索PubMed、Cochrane Library、Embase、CNKI、万方数据库、CBM数据库,按照纳入和排除标准选择临床研究,评价纳入文献质量并提取资料,采用RevMan 5.3软件进行Meta分析。结果:共纳入7篇病例对照研究,357例患者中颊黏膜尿道成形术组患者178例,舌黏膜尿道成形术组患者179例。Meta分析结果显示:两组手术成功率差异无统计学意义[OR=0.85,95%CI(0.48,1.51),P=0.57]。对手术后并发症和供区部位并发症定性评价显示:颊黏膜尿道成形术组患者易发生口周麻木、张口困难等术后并发症,舌黏膜尿道成形术组患者易发生发音改变等术后并发症。结论:颊黏膜尿道成形术和舌黏膜尿道成形术均是治疗前尿道狭窄的有效治疗方法,两者在手术成功率上无明显差异。舌黏膜取材较颊黏膜更加便利,可能更加有利于手术操作。 Objective:To compare the efficacy and safety between buccal mucosa graft(BMG)urethroplasty and lingual mucosa graft(LMG)urethroplasty for anterior urethral stricture with systematic review and meta-analysis.Method:We searched PubMed,Embase,CBM,Cochrane Library,CNKI,Wan Fang database from the inception to September 2015 for comparative studies assessing the efficacy and safety of BMG versus LMG in anterior urethral stricture.Result:Seven observational studies with a total of 357patients(178patients in BMG group,179 patients in LMG group)were included in this meta-analysis.There was no statistically significant difference in success rate[OR=0.85,95%CI(0.48,1.51),P =0.57]between BMG group and LMG group.Mouth tightness,perioral numbness were more common in BMG group,while slurred speech was more common in LMG group.Conclusion:It is concluded that the efficacy of LMG and BMG are similar.Compared with BMG,LMG is easier to harvest.
