摘要
目的调查湖南省0~6岁儿童脑性瘫痪（CP）流行特征及康复干预现状。方法于2011年9月至2012年9月期间,在湖南省长沙、岳阳、常德、郴州、怀化五地区采用分层整群随机抽样原则,抽取5个市共22个区乡镇1~6岁全体常住儿童作为调查对象,以问卷形式进行入户流行病学调查,采用Epidata3.2建立数据库,选用SPSS17.0版软件进行统计学分析。结果共实际调查湖南省五地区1~6岁儿童20 003例,发现脑瘫患儿53例,患病率为2.75‰;各地区的脑瘫患病率在2.09‰~3.25‰,各年龄组患病率分布在1.98‰~3.22‰;男女童脑瘫患病率之比为1.89∶1;CP分型以痉挛型为主（占58.18%）;脑瘫患儿中孕母孕期建立围生保健手册的占60.00%,接受了新生儿随访的患儿49.09%,脑瘫患儿中有47.27%选择医院集中式康复和社区康复结合的方式。结论湖南省0~6岁儿童脑瘫患病率为2.75‰,CP分型以痉挛型为主,围生期保健及早期随访工作尚待加强,康复干预现状有待进一步改善,可作为进一步研究脑瘫防治工作的基础性资料。
Objective： To discribe epidemiological features of cerebral palsy children aged 1 to 6 Years in Hunan Province. Methods： Children from five towns （Changsha, Yueyang, Changde, Chenzhou, Huaihua） aged 1 to 6 years were selected using stratified cluster sampling. A house-hold questionnaire survey was conducted from September, 2011 to September, 2012.The data were established by using Epidata3.2 software and analyzed by using SPSS 1 3.0 statistical analysis software. Results： Among 20 003 children aged 1-6 years surveyed.55 were found to have CP, a prevalence of 2.75‰.Prevalence in different towns range 2.09 ‰ to 3.25 ‰, the prevalence among all age groups range 1.98 ‰ to 3.22 ‰.Most （58.18%） had spastic CP. Only 60.00% mothers of cerebral palsy established perinatal care manual and guidance during pregnancy, 49.09% of them received neonatal follow up after birth, 47.27% of the children with CP had received hospital and community rehabilitation training.Conclusion： The prevalence of cerebral palsy among children aged 1 to 6 years is 2.35‰in Hunan Province, mainly of the spastic type. The prenatal care and early follow-up work is yet to be strengthened, rehabilitation intervention work needs further improvement, these basic data can be used for further research work to the prevention of cerebral palsy.
