频域光学相干断层扫描对自闭式小切口白内障摘除术后黄斑厚度的测量研究 被引量:1

Study on the macular thickness detected by spectral domain optical coherence tomography following the self-sealing small incision cataract extraction

摘要 目的观察自闭式小切口白内障摘除术后黄斑中心区厚度(CST)、黄斑容积(CV)、黄斑区视网膜平均厚度(CAT)的变化。方法接受自闭式小切口白内障摘除术且无手术并发症的单纯性老年性白内障患者30例(32眼),分别于术前及术后1 d、1周、1月进行频域光学相干断层扫描(Cirrus HD-OCT)检查,观察CST、CV、CAT的情况。结果所有术眼术后最佳矫正视力为0.5~1.0,均较术前提高,眼底镜检查未见黄斑的临床显性水肿,术前及术后1 d、1周及1月进行HD-OCT检查,所有术眼黄斑视网膜形态无明显异常,视网膜各层结构排列有序。术后1 d、1周的CST、CV、CAT较术前差异均无统计学意义(P均〉0.05);术后1月的CV、CAT较术前增加(P均〈0.05),CST无明显增加(P〉0.05)。结论无明显并发症的自闭式小切口白内障摘除术可引起黄斑厚度及容积的增加,CAT、CV的检测可比单纯CST的检测更早、更全面地反映出患者白内障术后视网膜厚度增加的情况,但所有患者黄斑厚度的增加均未引起明显的临床症状及黄斑水肿。 Objective To observe the changes of macular central subfield thickness ( CST), macular cube volume (CV) and macular cube avarage thickness (CAT) after self-sealing small incision cataract extraction. Methods Pro- spective study was carried out in 30 patients with simple age-related cataract ( 32 eyes) who received self-sealing small inci- sion cataract extraction without complication. Cirrus HD-OCT was performed to observe CST, CV and CAT before operation or 1 day, 1 week and 1 month after operation. Results Best corrected visual acuity was improved and reached 0. 5-1.0 after operation in all patients and no maeular edema occurred. Macular region form and structure barely changed. The values of CST, CV and CAT 1 day and 1 week after the operation showed no differences compared with those before operation ( P〉 0. 05 ). 1 month after surgery, the values of CV and CAT were significantly higher than those of before surgery ( P〈0.05 ), while CST showed no significant change compared with that before operation. Conclusions Macular thickness and vol- ume could be increased after self-sealing small incision cataract extraction. CV and CAT might reflect the increase of retinal thickness of cataract patients after operation more promptly and thoroughly.
