儿童代谢综合征患者心血管结构和功能改变及相关影响因素研究 被引量:2

The study of alteration about cardiovascular structure and function and related influencing factors in children metabolic syndrome patients

摘要 目的探讨儿童代谢综合征(metabolicsyndrome,MS)患者早期动脉硬化、心肌肥厚及左心功能的改变并探究三者改变的相关影响因素。方法MS组患儿70例,对照组儿童47例,分析MS组与对照组间颈动脉内中膜厚度(cIMT)、左心室质量指数(LVMI)、左心Tei指数的差异,评估三者之间的相关性以及与临床数据的相关性,并通过多元逐步回归,分析相关因素中的独立高危因素。结果①MS组与对照组之间cIMT、LVMI、左心Tei指数[(0.59±0.06)mm对(0.48±0.06)mm;(65.91±11.60)g/m2对(52.75±13.10)g/m2;0.33±0.04对0.28±0.02]的差异有统计学意义(P均〈0.05);②MS组cIMT与BMI、稳态胰岛素抵抗指数(HOMA—IR)、收缩压(SBP)呈正相关(r=0.741、0.550、0.519,P均〈0.05),BMI可作为cIMT的独立高危因素;③儿童MS组LVMI与cIMT、HOMA—IR、BMI、SBP呈正相关(r=0.798、0.772、0.679、0.394,P均〈0.05),HOMA—IR、cIMT可作为LVMI的独立高危因素;④儿童MS组左心Tei指数与LVMI、BMI、cIMT、HOMA-IR呈正相关(r=0.859、0.823、0.776、0.747,P均〈0.05),其中BMI、LVMI可作为左心Tei指数的独立高危因素。结论本组MS患儿已经出现心血管结构和功能受损的表现,且动脉硬化、心肌肥厚及左心功能受损三者改变相互平行影响。在疾病的初始阶段密切关注患者的体重、HOMA—IR,对降低心血管疾病的发病风险具有一定的临床意义。 Objective To discuss the alteration of early atherosclerosis, left ventricular myocardial hypertrophy and left heart function, and study related influencing factors about them in children metabolic syndrome(MS) patients. Methods The differences between the children MS group and control group in the carotid artery intima-media thickness (cIMT), left ventricular mass index (LVMI), left heart Tel index were analyzed. The correlation between cIMT, or LVMI with left heart Tel index, and the correlation between cIMT, LVMI,or Tei index with clinical data were evaluated. Results ①The differences of cIMT[(0.59 ± 0.06)mm vs (0.48 ± 0.06)mm], LVMI [( 65 . 91 ± 11.60)g/m2 vs (52.75 ± 13.10)g/m2], Tel index (0.33±0.04 vs 0. 28 ± 0. 02) between MS group and control group were found (P 〈0.05). ②cIMT in MS syndrome group showed a positive correlation with body mass index (BMI), homeostasis insulin resistance(HOMA-IR),and systolic blood pressure(SBP)( r = 0.741,0.550,0.519, P 〈0.05). BMI was an independent high risk factor of cIMT. ③LVMI in MS group performed a positive correlation with elMT, HOMA-IR, BMI, SBP (r = 0.798, 0.772, 0.679, 0.394, P〈 0.05). HOMA-IR and cIMT were independent high risk factors of LVMI. ④Tei index in the MS group demonstrated a positive correlation with LVMI, BMI, cIMT, HOMA-IR ( r = 0.859, 0.823, 0.776, 0.747, P〈 0.05). BMI, LVMI were independent high risk factors of Tel index. Conclusions The children of MS in the present study have emerged the change of cardiovascular structure and function. Moreover, the changes of clMT, LVMI and left heart Tei index were affected each other. In early stage of disease, to the body weight and insulin resistance index should be pay close attention to reduce the incidence of cardiovascular disease.
