重度宫腔粘连分离术后应用双节育器加防粘连膜预防宫腔再粘连的效果分析 被引量:17

摘要 目的 评价重度宫腔粘连(IUA)宫腔镜分离术后双节育器加防粘连膜治疗的临床效果.方法 选择48例重度IUA行宫腔镜电切分离术,所有患者按随机数字表法分为A、B、C三组(每组16例),A组术中放置两个节育器+防粘连膜,B组术中放置单个节育器+防粘连膜,C组术中放置单个节育器.术后患者均口服戊酸雌二醇5 mg/次,1次/12 h,行人工周期治疗,后10 d加服安宫黄体酮片10 mg/d.患者术后予规范记录月经量,术后3个月复查宫腔镜评估宫腔情况.结果 治疗后,A组治愈率明显高于B、C组(x2=7.38,16.76,P<0.05或P<0.01),B组治愈率虽高于C组,但差异无统计学意义(P>0.05);A组治疗总有效率明显高于C组(x2 =5.33,P<0.05),B组总有效率与C组比较差异无统计学意义(P>0.05);三组患者中闭经者术后月经均有不同程度恢复,与治疗前月经量比较差异均有统计学意义(t=13.15,15.28,15.44,P <0.05),A组月经量明显多于B、C组(F=8.65,16.85,P<0.05);B组月经量明显多于C组(F =7.16,P<0.05),且无明显不良反应.结论 宫腔镜分离重度IUA术后应用双节育器加防粘连膜治疗效果肯定,此方法能有效建立良好的物理屏障,有助于防止术后复发粘连,值得临床推广应用.
