摘要
目的观察口腔幽门螺杆菌清除对HP阳性患者的口腔HP阳性率、胃HP根除率、用药安全性和胃HP复发率的影响。方法将我科2014年1月至2015年1月收治的同时具有口腔和胃HP感染阳性的216例消化系统疾病的患者随机分为研究组和对照组,每组患者数均为108例,对照组采用单纯序贯疗法进行治疗,治疗组采用序贯疗法联合口腔洁治进行治疗,疗程均为10d。观察两组治疗期间的用药安全性和治疗后1个月的口腔HP阳性率和胃HP根除率;随访1年,比较6个月、1年的口腔HP阳性率和胃HP复发率。结果在治疗结束后1个月,研究组的口腔HP阳性率为34.26%明显低于对照组的58.33%(P=0.000),而研究组的胃HP根除率为91.67%明显高于对照组的70.37%(P=0.000);在治疗后6个月,研究组的口腔HP阳性率和胃HP复发率分别为41.67%和13.89%,均明显低于对照组的66.67%和34.26%(P=0.000,0.001);在治疗后1年,研究组的口腔HP阳性率和胃HP复发率分别为49.07%和25.00%,均明显低于对照组的82.41%和45.37%(P=0.000,0.003);对照组治疗后1年的口腔HP阳性率明显高于治疗后6个月(P<0.05)。两组均未发生严重不良反应。结论口腔是HP感染的重要场所,口腔HP清除可以通过避免交叉感染从而降低胃HP清除率以及复发率。
Objective To observe the effect of oral helicobacter pylori clearance on the positive rate of oral HP, the eradicate rate of gastric HP,drug safety and the recurrence rate of gastric HP in HP positive patients. Methods 216 cases of patients admitted to digestive system department between January 2014 and January 2015 with digestive system diseases with HP positive both in mouth and stomach were randomly and equally divided into research group and the control group. The control group was treated with simple sequential therapy, while the research group was treated with sequential therapy combined with oral cavity clearance therapy for the same course of 10 d. The drug safety during treatment, the positive rate of oral HP and eradicate rate of gastric HP 1 month after treatment were observed in two groups ; the positive rate of oral HP and eradicate rate of gastric HP for 6 months and 1 year were compared after a for 1 -year-followed up.
