摘要
目的调查医院超说明书用药情况,为制订及改进超说明书用药管理措施提供参考。方法采用超说明书用药专项点评的方法,分析超说明书用药的合理性与必要性,对超说明书用药的管理与持续改进提出整改意见。结果超说明书用药的主要体现:临床应用广泛,有新的治疗指南或专家共识支持;无大样本、多中心循证医学支持,但有相关文献可循,且无合理的可替代药品(此两类均属合理用药,在医院使用广泛,但需加强用药管理);药品使用依据不足,应禁止其超说明书用药(此类现象已逐步被控制)。结论医院应加大对超说明书用药管理的力度,完善超说明书用药专项点评标准,加强医、药、护之间的沟通与合作,减少不合理超说明书用药,使超说明书用药更加合理化与规范化,保障临床安全、有效、合理用药。
Objective To investigate the off- label drug use in the hospital,to provide reference for formulating management measures of off- label drug use. Methods The rationality and necessity of off- label drug use were analyzed with the special prescription audit, and the recommendations and suggestions on management and continuous improvement were put forward. Results There were 2 kinds in off-label drug use： extensive clinical application with new treatment guidelines or expert consensus; no large sample multicenter evi- dence- based support, but there was literature to follow and no reasonable alternative medicines. These 2 kinds were rational drug use and were widely used in the hospital, but we still needed to strengthen the drug management; if the drug usage evidence was inade- quate, its usage should be banned, and this phenomenon had been radually controlled in the hospital. Conclusion The hospital should increase management efforts on the off- label drug use, improve the special prescription comment standards of the off- label drug use, strengthen communication and cooperation among doctors, pharmacists and nurses,reduce the irrational medication, to make the off-label drug use ration and standard,thus ensure the clinical safe,effective and rational drug use.
