摘要
目的探讨知柏地黄丸联合盐酸克林霉素对牙周炎的临床疗效及对血清白介素-8(interleukin-8,IL-8)、肿瘤坏死因子α(tumor necrosis factorα,TNF-α)、白介素-10(IL-10)水平的影响。方法回顾性选取绍兴市人民医院口腔科确诊为牙周炎患者98例作研究对象,按随机数字表法进行分组,对照组49例在基础治疗上予以盐酸克林霉素治疗;研究组49例在基础治疗上予以盐酸克林霉素联合知柏地黄丸治疗,治疗前后检查牙周袋深度、菌斑指数、牙龈指数及探针出血阳性率,采血测定血清IL-8、TNF-α、IL-10水平,同时对比临床疗效。结果研究组治疗有效率(93.88%)高于对照组治疗有效率(79.59%),差异具有统计学意义(P<0.05)。观察组探诊出血阳性率(38.78%)显著低于对照组为(59.18%),差异具有统计学意义(P<0.05)。研究组治疗后牙周袋深度、菌斑指数、牙龈指数低于对照组,研究组治疗后TNF-α和IL-8水平低于对照组,IL-10高于对照组,差异具有统计学意义(P<0.05)。结论采用盐酸克林霉素联合知柏地黄丸治疗牙周炎疗效更好,能降低牙周炎血清IL-8、TNF-α水平,提高IL-10水平。
Objective To investigate the effect of Zhibaidihuangwan combined with clindamycin hydrochloride on serum IL-8,TNF-α,IL-10 levels in periodontitis and its clinical efficacy. Methods A total of 98 patients with periodontitis from our hospital were collected and randomly divided into the control group and the experimental group,49 cases each group. Patients in the control group were treated on the basis of the foundation therapy with clindamycin hydrochloride,patients in the experimental group were treated with Zhibaidihuangwan on the basis of control group. The periodontal pocket depth,plaque index,gingival index and positive rate of probe bleeding,serum IL-8,TNF-a,IL-10 levels were measured,and the clinical efficacy were compared. Results The effective rate of the experimental group（ 93. 88%） was higher than the control group（ 79. 59%）,with statistical significance（ P〈0. 05）. The positive rate of probe bleeding in experimental group（ 38. 78%） was lower than control group of two groups（ 59. 18%）,with statistical significance（ P〈0. 05）. The periodontal pocket depth,plaque index,gingival index in experimental group were lower than the control group after treatment,the TNF-α and IL-8 levels in experimental group were lower than the control group,IL-10 was higher than the control group after treatment,with statistical significance（ P〈0. 05）. Conclusion Zhibaidihuangwan combined with clindamycin hydrochloride could reduce periodontitis serum IL-8 and TNF-α levels,improve the level of IL-10.
