摘要
目的观察氟伏沙明联合羟考酮缓释片治疗中重度癌痛合并抑郁的临床效果及安全性。方法选取江苏省泰兴市人民医院2013年1月至2014年9月收治的中重度癌痛合并抑郁的患者120例，按随机数字表法分为对照组和观察组，各60例。分别口服羟考酮缓释片（滴定剂量）和羟考酮缓释片（滴定剂量）＋氟伏沙明片（首剂：50mg／次，1次／d，隔日增加50～100mg，根据疗效调整剂量，不超过150mg／次，2次／d）。比较2组疼痛控制稳定后的数字疼痛强度分级法（NRS）评分和治疗4周后抑郁自评量表（SDS）评分以及不良反应发生率。结果治疗后，与对照组相比较，观察组NRS评分、SDS评分更低，差异有统计学意义[（1．5±1．1）分比（2．1±1．1）分、（42±7）分比（46±8）分，均P〈0．05]；观察组便秘发生率低于对照组，差异有统计学意义[31．7％（19／60）比51．7％（31／60），P〈0．05]，其余不良反应发生率2组差异无统计学意义（P〉0．05）。结论氟伏沙明片联合羟考酮缓释片能有效控制癌痛，缓解抑郁症状，减少羟考酮缓释片的部分不良反应。
Objective To observe the clinical efficacy and security of oxycodone hydrochloride controlled- release tablets combined with fluvoxamine in treating moderate and severe cancer pain complicated with depression. Methods One hundred and twenty patients with moderate and severe cancer pain complicated with depression from January 2013 to September 2014 were randomly divided into control group given oxycodone hydrochloride controlled-release tablets （titrating dosage） and observation group given oxycodone hydrochloride controlled-release tablets （titrating dosage） and fluvoxamine （first dose 50 mg/time, increased by 50-100 mg next day, ≤300 mg/d）. The pain numerical rating scale （NRS） score, self-rating depression scale （SDS） score, incidence of adverse reactions 4 weeks after treatment were compared between groups. Results After treatment, the NRS score and SDS score in observation group were significantly lower than those in control group [ （ 1.5 ± 1.1 ） scores vs （2. 1 ± 1.1 ） scores, （42± 7） scores vs （46 ±8 ） scores ] （ P 〈 0, 05 ）. The incidence of constipation in observation group was significantly lower than that in control group [31. 7% （19/60） vs 51.7% （31/60）] （P 〈0.05）, the incidences of other adverse effects were not significantly different between groups （P 〉 0. 05）. Conclusion Fluvoxamine combined oxycodone hydrochloride controlled-release tablets can effectively control the cancer pain, relieve the symptom of depression, and reduce the adverse reactions induced by oxycodone hydrochloride controlled-release tablets.
