摘要
目的探讨E钙黏蛋白和基质金属蛋白酶-9(MMP-9)在盆底器官脱垂(POP)患者子宫骶韧带中的表达情况.并分析两者的相关性。方法选取2015年1月~2016年1月武汉大学人民医院妇产科子宫全切患者55例,分为三组,其中轻度POP组(POPⅠ-Ⅱ度)15例,重度POP组(POP Ⅲ~Ⅳ度)22例,正常组(非POP患者)18例,采用蛋白质印迹法检测三组患者子宫骶韧带组织中E-钙黏蛋白和MMP-9的表达情况,比较分析两者在各组的相关性。结果轻度POP组及重度POP组的E-钙黏蛋白表达显著低于正常组(P〈0.01).而MMP-9的表达高于正常组(P〈0.05或P〈0.01),并且E-钙黏蛋白的降低和MMP-9的升高随POP严重程度加重而加大;E-钙黏蛋白和MMP-9在轻度和重度POP组的表达呈负相关(r=-0.5612,P=0.0295;r=-0.6221,P=0.002)。结论E-钙黏蛋白降低和MMP-9的升高可能是POP发病机制之一.二者可能共同参与了POP的发生发展。
Objective To investigate expression of E-cadherin and matrix metalloproteinase-9 (MMP-9) in uterosacral ligament of women with pelvic organ prolapse and analyze the correlation between the two proteins. Methods Fifty-five patients undergoing hysterectomy were selected from January 2015 to January 2016 in the Department of Gynecology and Obstetrics of Renmin Hospital of Wuhan University, then they were divided into three groups: the mild group (POP Ⅰ -Ⅱ, 15 cases), the severe group (POP Ⅲ-Ⅳ, 22 cases) and control group (without POP, 18 cases). Western blot was used to detect E-cadherin and MMP-9 of the uterosacral ligament tissues taken from patients of the three groups, then the differences and correlation of the expression of both proteins were analyzed among the three groups. Results The level of E-cadherin in the mild group and the severe group was lower (P 〈 0.01) and MMP-9 was higher than those of the control group (P 〈 0.05 or P 〈 0.01), the decrease of E-cadherin and the rise of MMP-9 increased with severity increase of POP. There was a significant negative correlation between the two proteins in the mild group and the severe group (r = -0.5612, P = 0.0295; r = -0.6221, P = 0.002). Conclusion The down-regulation of E-cadherin and up-regulation of MMP-9 in uterosacral ligament tissues may be one of the pathogenesis of POP. The both proteins may paticipate in the pathogenesis of POP.
