血浆脑钠肽前体和降钙素原水平检测在机械通气脱机中的应用分析 被引量:18

摘要 目的探讨血浆脑钠肽前体(pro-brain natrituretic peptide,pro-BNP)和降钙素原(procalcitonin,PCT)水平的检测在机械通气脱机中的应用效果分析。方法我院在2014年1月至2015年12月收治的呼吸衰竭给予机械通气治疗的患者中,选择具有可比性的80例作为研究对象,所有研究对象均在达到自主呼吸试验标准后给予脱机,根据研究对象的脱机情况将其分成成功组与失败组,分别对比两组患者的血浆pro-BNP水平、PCT水平、氧合指数、平均心率以及平均动脉压等各项指标,同时对影响机械通气脱机成功的独立危险因素进行分析。结果成功组血浆pro-BNP、PCT检测结果均低于失败组,而氧合指数高于失败组,且差异均有统计学意义(P均<0.05),平均心率及平均动脉压两组间差异均无统计学意义(P均>0.05);血浆PCT、pro-BNP均为影响机械通气脱机成功的独立危险因素。结论 pro-BNP、PCT水平能有效反应机械通气患者的脱机风险,在给予自主呼吸试验时联和检查pro-BNP、PCT水平,能显著提高机械通气患者的脱机成功率。 Objective To investigate the pro-brain natriuretic peptide (pro-BNP) and procalcitonin (PCT) levels detected in the application effect of mechanical ventilation in the offline analysis. Methods 80 patients in our hospital from January 2014 to December 2015 admitted with respiratory failure treated with me-chanical ventilation therapy were collected as research subjects. All subjects were given standard tests to achieve spontaneous breathing off machine , and which were divided into success group and failure group ac-cording offline research object. The levels of plasma pro-BNP, PCT, oxygenation index, average heart rate and mean arterial pressure and other indicators of all the patients were detected , and independent risk factors for the success of mechanical ventilation offline were analyzed. Results The levels of pro-BNP and PCT in success group were all lower than that of failure group , but oxygenation index was higher than that of failure group, and the differences all had statistical significance(Pall〈0.05). The differences of average heart rate and mean arterial pressure of the two groups had no statistical significance (Pall〉0.05). Pro-BNP and PCT were all independent risk factors for mechanical ventilation offline success. Conclusion The plasma pro-BNP and PCT levels can respond effectively offline risk mechanically ventilated patients , administration of a sponta-neous breathing trial linking and inspection of pro-BNP and PCT levels can be significantly improved mechan-ically ventilated patients offline success rate.
