摘要
目的：探讨以晶状体超声乳化吸出联合人工晶状体植入及睫状体平坦部玻璃体抽吸手术治疗睫状环阻塞性青光眼的临床效果。方法观察闭角型青光眼小梁切除术后，发生经药物治疗不能缓解的睫状环阻塞性青光眼且合并晶状体浑浊10例（10眼），实施晶状体超声乳化吸出联合人工晶状体植入及睫状体平坦部玻璃体抽吸手术，术后随访4～6个月，对手术前后的视力、眼压、前房深度及前房角进行比较分析。结果术后所有患者的视力、眼压、前房深度、前房角均有不同程度的改善。术后1例虹膜后粘连，2例瞳孔区渗出膜，术后10 d用Nd：YAG激光切开人工晶状体前表面渗出膜。1例术后前房浅，眼压28．0 mmHg（1 mmHg＝0．133 kPa）；应用散瞳及皮质类固醇药物治疗效果不明显，术后5 d行Nd：YAG激光后囊切开及中央玻璃体前界膜切开后，前房形成，术后随访6个月，眼压控制良好。结论晶状体超声乳化吸出联合人工晶状体植入及睫状体平坦部玻璃体抽吸术，可有效治疗睫状环阻塞性青光眼。
of IOL and pars plana vitreous aspiration in the treatment of ciliary block glaucoma after trabeculectomy . Methods Ten eyes of ten cases of ciliary block glaucoma with cataract after trabeculectomy were treated in our hospital .All the 10 eyes received phacoemulsification with implantation of IOL combined with pars plana vitreous aspiration .After 4-6 months of follow-up, the visual acuity , intraocular pressure , anterior chamber depth, anterior chamber angle and postoperative complications were analyzed .Results After surgery the visual acuity , intraocular pressure , anterior chamber depth , and anterior chamber angle in the ten patients were improved by different degrees .Iris synechia occurred in one eye .Two eyes with exudtive papillary membrane had undergone Nd：YAG laser capsulotomy on the 10th day after operation.One eye with shallow anterior chamber and high IOP （28.0 mmHg）, with negative responding to mydriatic and corticosteroids , underwent Nd：YAG laser periphery capsulotomy and hyaloidectomy on the 5th day after operation and the IOP was in good control after 6 months follow-up.Conclusion Phacoemulsification combined with implantation of IOL and pars plana vitreous aspiration is effective in the treatment of ciliary block glaucoma .
