大肠腺癌中单核细胞趋化蛋白-1的表达及意义

Expression and significance of monocyte chemoattractant protein-1 in colorectal carcinoma

摘要 目的探讨单核细胞趋化蛋白-1(MCP-1)在大肠腺癌组织中的表达及与临床病理特征的关系。方法采用免疫组织化学S-P法检测38例大肠腺癌组织、22例大肠腺瘤组织、29例正常大肠组织中MCP-1的表达情况。结果 MCP-1在大肠腺癌组织中的阳性表达率为71.1%(27/38),在大肠腺瘤组织中的阳性表达率为36.4%(8/22),在正常大肠组织中的阳性表达率为20.7%(6/29)。MCP-1在大肠腺癌组织中的阳性表达率明显高于大肠腺瘤组织与正常大肠组织,差异均具有统计学意义(P〈0.05),大肠腺瘤组织与正常大肠组织间MCP-1阳性表达率比较差异无统计学意义(P〉0.05)。肿瘤浸润达浆膜或浆膜外MCP-1阳性表达率为88.9%,高于肿瘤浸润未及浆膜的27.3%(P〈0.05);有淋巴结转移MCP-1阳性表达率为93.8%,高于无淋巴结转移的54.5%(P〈0.05);TNM分期Ⅲ~Ⅳ期的MCP-1阳性表达率为93.8%,高于TNM分期Ⅰ~Ⅱ期的54.4%(P〈0.05)。结论在大肠腺癌组织中MCP-1的表达明显高于大肠腺瘤组织和正常大肠组织,提示MCP-1参与大肠腺癌的发展;MCP-1的阳性表达率与肠癌的肠壁浸润深度、淋巴结转移、TNM分期相关,提示MCP-1表达与大肠腺癌的浸润、转移及分期有关。 Objective To investigate expression of monocyte chemoattractant protein-1(MCP-1) in colorectal carcinoma and its relationship with clinical pathological characteristics. Methods immunohistochemistry S-P test was applied to detect MCP-1 expression in 38 colorectal carcinoma tissues, 22 colorectal adenoma tissues and 29 normal intestine tissues. Results MCP-1 showed positive expression rate as 71.1%(27/38) in colorectal carcinoma tissues, 36.4%(8/22) in colorectal adenoma tissues and 20.7%(6/29) in normal intestine tissues. The positive expression rate of MCP-1 in colorectal carcinoma tissues was obviously higher than in colorectal adenoma tissues and normal intestine tissues, and their differences all had statistical significance(P〈0.05). There was no statistically significant difference of positive expression rate of MCP-1 between colorectal adenoma tissues and normal intestine tissues(P〉0.05). The positive expression rate of MCP-1 by serosa or tunica serosa tumor infiltration was higher as 88.9% than 27.3% by non-serosa tumor infiltration(P〈0.05). The positive expression rate of MCP-1 with lymphatic metastasis was higher as 93.8% than 54.5% without lymphatic metastasis(P〈0.05). The positive expression rate of MCP-1 in TNM staging Ⅲ ~ Ⅳ was higher as 93.8% than 54.4% in TNM staging Ⅰ ~ Ⅱ(P〈0.05). Conclusion Higher MCP-1 expression in colorectal carcinoma tissues than colorectal adenoma tissues and normal intestine tissues reveals involvement of MCP-1 in development of colorectal carcinoma. Positive expression of MCP-1 is related with intestinal infiltration depth, lymphatic metastasis and TNM staging, and it shows relationship between MCP-1 expression and infiltration, metastasis and staging.
