摘要
[目的]"痿"最早见于《黄帝内经》,《素问·痿论》详细阐发机理,成为后世医家研讨基础,历代医家看法大同小异,均认为以筋肉无力,肢体不能随意运动为主要表现,易与痹症混淆,吴昆提到"痿痹二病也,《内经》亦有称痹为痿者,故而为一",张子和认为:"四末之疾,动而或痉者为风,不仁或痛者为痹,弱而不用者为痿,逆而寒热者为厥,此其状未尝同也"。痿病包涵现代医学多种疾病(重症肌无力,运动神经元病等),皆影响到神经、肌肉等组织。痹症属风湿类疾病,也有相当一部分影响到神经、肌肉;而归为痿病范畴的重症肌无力从西医角度来讲是以体液免疫为主的神经系统自身免疫性疾病,同样的,一些归为痹症范畴的风湿类疾病与自身免疫失调有关。痿病成因不仅在于情志内伤等,亦与外伤邪气有关,痿症以肺热叶焦、阳明虚、带脉不引、内虚、湿热为主要病机。
Atrophy-syndrome was first seen in the Huangdi Neijing,in the Atrophy-syndrome Theory of Suwen, it＇s a comprehensive description of the etiology and pathogenes,later physicians developed of the theory of Atrophy-syndrome disease on the basis of the research, doctors in past dynasties have the similar view and incline that main performance include muscle weakness, the limbs are not free to move, easy to confuse with arthromyodynia. WU Kun proposed that ＂Atrophy-syndrome and arthromyodynia are different diseases, NeiJing sometimes called arthromyodynia as Atrophy-syndrome, so and for one＂ . ZHANG Zihe proposed that ＂Limbs disease, movement and convulsion is wind, numbness or pain is arthromyodynia, weakness and unable is Atrophy-syndrome, inverse to chills and fever is syncope syndrome, their symptoms are not the same＂. Atrophy- syndrome includes a variety of diseases of modern medicine, （myasthenia gravis, motor neuron disease, etc.） All of the diseases affect the nerve, muscle etc. Arthromyodynia belongs to rheumatic diseases, considerable part of it also affect the nerves and muscles; and myasthenia gravis divided into atrophy-syndrome is autoimmune disease of the nervous system, which is mainly based on humoral immunity from the point of view of western medicine, likewise, some rheumatic diseases divided into arthromyodynia are associated with autoimmune disorders. The causes of Atrophy-syndrome is not only emotional injuries, but also related to exogenous pathogenic factors etc. lung heat scorching, deficiency of Yangming meridian, dai meridian without constraint, endopenia and damp- heat are the main pathogenesis of Atrophy-syndrome.
