摘要
目的对该院门诊药房不适宜用药干预进行统计分析,加强用药安全管理。方法回顾分析该院2013年5月—2014年5月不适宜用药干预记录表,对不适宜用药干预记录表进行统计分析。结果对该院门诊药房所登记的1 684例不适宜用药干预记录进行分析。用法用量错误1 082张,占比64.25%;有潜在临床意义的药物相互作用和配伍禁忌167张,占比9.92%;遴选的药品不适宜65张,占比3.86%;重复给药47张,占比2.79%;必须做皮试的药品患者未做皮试187张,占比11.1%;适应症不适宜136张,占比8.08%。结论门诊药房用药存在用药不合理情况主要表现在重复用药和用法用量等方面,临床医生应与门诊药房药师加强沟通,努力做到合理用药。
Objective To statistically analyze the dispensary for outpatients not suitable for medication intervention in our hospital and enhance the medication safety management. Methods The record charts not suitable for medication intervention in our hospital from May 2013 to May 2014 were retrospectively analyzed and statistically analyzed. Results 1 684 cases of record charts not suitable for medication intervention registered in the dispensary for outpatients in our hospital were ana-lyzed, there were 1 082 with mistake dosage and administration, accounting for 64.25%, there were 167 with interaction and incompatibilities of drugs with potential clinical significance, accounting for 9.92%, there were 65 with inappropriate selec-tive drugs, accounting for 3.86%, there were 47 with repeated administration, accounting for 2.79%, there were 187 without skin test of patients who must have a skin test, accounting for 11.1%, and there were 136 with inappropriate indication, ac-counting for 8.08%. Conclusion The main manifestations of rational medication in dispensary for outpatients are the re-peated medication and dosage and administration, the clinical doctor should enhance communication with the pharmacists in dispensary for outpatients and make effort for rational administration.
