摘要
目的分析重症急性胰腺炎(SAP)合并感染患者应用肠内外营养治疗的效果,为临床治疗提供参考依据。方法应用随机数字表法将2013年1月-2015年9月就诊于医院66例SAP合并感染患者分为两组,接受肠外联合肠内营养治疗患者为肠内组,接受肠外营养治疗患者为肠外组,每组各33例;对比两组患者感染率、营养指标改善及免疫指标改善等。结果接受治疗7d后,肠内组患者WBC、N%及CRP明显低于肠外组,差异有统计学意义(P<0.05);两组患者治疗前及治疗7d后白蛋白(ALB)、前蛋白(PA)及血红蛋白(Hb)比较,差异均无统计学意义;两组间治疗前IgA、IgG、IgM比较,差异均无统计学意义,治疗7d后,肠内组患者IgA、IgG、IgM明显高于肠外组,差异有统计学意义(P<0.05)。结论肠外联合肠内营养治疗,可有效改善SAP合并感染患者病情及免疫状况,可作为常规营养方案应用于SAP合并感染患者的治疗方法。
OBJECTIVE To observe the effect of enteral and parenteral nutritional support treatment on severe acute pancreatitis（SAP）patients complicated with infections so as to provide guidance for clinical treatment.METHODS A total of 66 SAP patients complicated with infections who were treated in the hospital from Jan 2013 to Sep 2015 were enrolled in the study and randomly divided into the enteral group and the parenteral group,with 33 cases in each.The enteral group was given parenteral nutritional support treatment combined with enteral nutritional support treatment,while the parenteral group was given parenteral nutritional support treatment.The infection rate,improvement of nutrition indicators,and improvement of immune indexes were observed and compared between the two groups of patients.RESULTS The WBC,N%,and CRP were significantly lower in the enteral group than in the parenteral group after the treatment for 7days（P〈0.05）.There was no significant difference in the albumin（ALB）,prealbumin（PA）,or hemoglobin（Hb）between the two groups of patients before the treatment and after the treatment for 7days.There was no significant difference in the IgA,IgG,or IgM between the two groups of patients before the treatment;the IgA,IgG,and IgM were significantly higher in the entered nutritional parenteral nutritional group than in the control group after the treatment for 7days（P〈0.05）.CONCLUSION The enteral nutritional support treatment combined with parenteral nutritional support treatment can effectively improve the illness condition and immune state of the SAP patients complicated with infections,which can be used as conventional nutrition program for treatment of the SAP patients complicated with infections.
