摘要
目的探讨脂联素与慢性乙型病毒性肝炎患者肝纤维化的相关性。方法纳入30例肝纤维化患者、30例肝硬化患者、30例健康体检者为研究对象,对肝纤维化及肝硬化患者以恩替卡韦抗病毒,分别于纳入研究当日、24周、48周检测所有对象肝功能、脂联素水平、肝纤四项。比较各组脂联素水平;评价脂联素与AST/ALT、TBA、肝纤维化指标的相关性;单独以肝纤维化及肝硬化患者为研究对象,将48周时肝纤维化得到改善患者纳为改善组,其余患者纳为未改善组,对比血清脂联素水平。结果 1各时点对比,血清脂联素水平均呈健康体检者<肝纤维化组<肝硬化组,组间差异均有统计学意义(P<0.05);2脂联素水平与AST/ALT、TBA、PCⅢ、LN、CⅣ、HA均呈正相关(P<0.05);3改善组血清脂联素水平明显低于对照组(P<0.05)。结论血清脂联素水平随肝纤维化、肝硬化病程发展而上升;脂联素水平与肝功能恶化程度、肝纤四项均有明显相关性;患者若接受抗病毒治疗,脂联素水平将显著下降。
Objective To investigate the correlation between adiponectin and liver fibrosis in patients with chronic hepatitis&amp;nbsp;B.Methods Totally 30 cases of patients with hepatic fibrosis,30 patients with cirrhosis and 30 healthy subjects were enrolled in the study.Liver fibrosis and cirrhosis patients accepted antiviral therapy with entecavir.All subjects were included in the study on the same day,24 weeks,48 weeks to detect all objects of liver function,serum adiponectin levels and hepatic fibrosis.The correlation between adiponectin and AST/ALT,TBA,and liver fibrosis indexes were compared between the groups.The patients with hepatic fibrosis and cirrhosis were divide into improvement group and non -improvement group and the serum adiponectin levels were compared.Results ①At different time points,the levels of serum adiponectin was highest in healthy people,fol-lowed by hepatic fibrosis group and hepatic cirrhosis.,and the difference was statistically significant (P 〈0.05);②Adiponectin levels were positively correlated with AST/ALT,TBA,PC,LN,CⅣ,and HA (P 〈0.05);③The serum adiponectin level in the improvement group was significantly lower than that in the control group (P 〈0.05).Conclusion The level of serum adi-ponectin is increased with the development of hepatic fibrosis and cirrhosis;adiponectin levels are significantly correlated with the degree of deterioration of liver function and liver fibrosis,and the level of adiponectin is significantly decreased in patients who receive antiviral therapy.
