冠心病心力衰竭患者血尿酸、血浆脑利钠肽与左室射血分数的相关性分析 被引量:1

摘要 目的探讨冠心病心力衰竭患者血尿酸水平与血浆脑钠肽水平的相关性及与左室射血分数的关系。方法随机选取我院心血管内科住院的80例冠心病心力衰竭患者作为观察组,对照组为非心血管疾病患者30例。两组患者均行血尿酸、血浆脑利钠肽及左室射血分数检测并进行比较,分析尿酸、脑利钠肽与左室射血分数的关系及血尿酸与脑利钠肽的相关性。结果观察组血尿酸、脑利钠肽明显高于对照组[(486.56±78.4)μg/ml比(320.12±82.24)μg/ml,(984.2±482.6)μg/nd比(26.1±5.2)μg/ml](P〈0.05,P〈0.01),左室射血分数观察组低于对照组[(38±3.6)%比(55±4.5)%,P〈0.01)]。血尿酸及脑钠肽与左室射血分数呈负相关(r=-0.701,P〈0.01;r=-0.695,P〈0.01),尿酸与脑钠肽呈正相关(r=0.807,P〈0.01)。结论高血尿酸是冠心病心力衰竭的又一个独立的危险因素,它和血浆脑钠肽一样,可以反映心功能恶化程度。血尿酸及脑钠肽与左室射血分数呈负相关。 Objective To investigate distribution and relationship between uric acid (UA) and brain natriuretic peptide(BNP) and left ventricular ejection fraction(LVEF) in heart failure patients caused by coronary heart disease. Methods 80 patients of heart failure caused by coronary heart disease were randomly chosen as observation group, 30 patients without cardiovascular disease were chosen as control group. Serum BNP, uric acid concentration and LVEF of all patients was measured and compared. Results BBoth uric acid and BNP concentration were significant higher in observation group than the persons as control: UA[ (486.56±78.40)μg/ml vs (320.12±82.24)μg/ml ], BNP [ ( 984.2±482.6 )μg/ml vs ( 26.1 ±5.2 )μg/ml ] (P〈0.05, P〈0.01 ). LVEF values of observation groupwere significant lower than control group (EF 38.0%±3.6% vs 55.0%±4.5%,P〈0.01 ). Uric acid and BNP concentration were negative correlated with LVEF values(r=-0.701 ,P〈0.01 ,r=-0.695 ,P〈0.01 ). There was positive correlation between uric acid and BNP concentration(r=0.807,P〈0.01 ). Conclusion Serum BNP and uric acid concentration are positively correlated with LVEF values, serum BNP concentration is positively correlated with uric acid concentration. Uric acid may predict the severity of heart failure patients caused by coronary heart disease as BNP.
