COPD诱导痰细胞分类、降钙素原、C-反应蛋白检测的意义 被引量:3

Significance of detecting induced sputum cell classification,serum procalcitonin and C-reactive protein in chronic obstructive pulmonary diseases

摘要 目的:探讨不同期慢性阻塞性肺疾病( COPD)患者诱导痰中炎性细胞组成及血清降钙素原( PCT)、C-反应蛋白( CRP)的变化。方法对50例COPD急性加重期及稳定期患者分别进行超声雾化高渗盐水诱导咳痰,痰液处理后进行炎性细胞分类,并测定血清PCT、CRP的浓度。结果 COPD急性加重期与稳定期诱导痰中均以中性粒细胞为主,且急性加重期高于稳定期,而巨噬细胞分类前者低于后者,PCT、CRP浓度急性加重期高于稳定期。结论 PCT、CRP在COPD急性加重期较稳定期明显升高,提示细菌感染、全身炎症反应加重是AECOPD的重要诱因,相应诱导痰细胞学分类表现为中性粒细胞比例升高。 Objective To investigate the changes and significance of induced sputum cell classi-fication,serum procalcitonin and C-reactive protein in different stages of chronic obstructive pulmonary diseases( COPD). Methods Induced sputum cell classifications serum procalcitonin and C-reactive pro-tein of 50 cases of COPD were respectively detected in acute exacerbation and stable stages. Results The percentages of neutrophil in induced sputum were highest in both acute exacerbation and stable stage of COPD. The percentages of neutrophil were increased and macrophages decreased significantly in acute exacerbation of COPD than in stable stage. Serum procalcitonin and C-reactive protein were increased significantly in acute exacerbation of COPD than in stable stage. Conclusions Serum procalcitonin and C-reactive protein are increased significantly in acute exacerbation of COPD than in stable stage,which indicated that bacterial infections and systemic inflammatory response are causes of acute exacerbation of COPD,and the percentages of neutrophil in induced sputum are increased correspondingly.
