活动期类风湿关节炎患者血清IL-33和TNF-α变化的临床意义研究 被引量:3

摘要 目的 :通过检测活动期、缓解期类风湿关节炎(rheumatoid arthritis,RA)患者以及健康者血清中肿瘤坏死因子α(tumor necrosis factorα,TNF-α)和白细胞介素33(interleukin 33,IL-33)的质量浓度水平,探讨它们与RA的发生以及疾病活动度的关系。方法:收集活动期RA患者血清30例,缓解组血清30例,健康人血清30例,用酶联免疫吸附(enzyme-linked immunosorbent assay,ELISA)法检测血清中TNF-α和IL-33水平,并用统计学方法分析它们与相关RA实验室指标:抗环瓜氨酸(cyclic peptide containing citrulline,CCP)抗体、血沉(erythrocyte sedimentation rate,ESR)、类风湿因子(rheumatoid factor,RF)、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)和DAS28评分之间的关系。结果:RA疾病活动组中的IL-33及TNF-α质量浓度水平明显高于缓解组和正常对照组(P<0.05);RA疾病活动组和缓解组中的TNF-α和IL-33质量浓度水平呈直线正相关关系(r=0.736、0.512,P<0.05);RA疾病活动组患者血清中IL-33质量浓度水平与抗CCP抗体、RF呈正相关关系(P<0.05);RA活动组患者血清中TNF-α水平与DAS28评分和ESR呈正相关(P<0.05)。结论 :IL-33和TNF-α在RA发病中发挥着重要作用,两者与RA的发生与发展关系密切,它们不仅参与了RA患者的发病进程,而且与疾病的活动度呈正相关,尤其是血清中TNF-α的表达水平有望成为RA活动的评价指标之一。 Objective: To determine the serum levels of tmnor necrosis factor α(TNF-α) and interleukin 33(IL-33) in rheumatoid arthritis(RA) further analysis laboratory indexes with their relationships and to assess their clinical significance. Methods: 60 serums of RA patients, 30 patients with active RA and 30 patients with disease in remission were collected. Serum of thirty healthy people were recruited as controls. Concentrations of TNF-α and IL-33 in serum were measured by enzyme-linked immunosorbent assay(ELISA), and statistical methods were used to analyse the relationship between them and the related clinical and laboratory data. Results: The levels of serum TNF-α and IL-33 in active RA group are obviously higher than the RA group in remission stage and the normal control group, the difference was statistically significant(P〈0.05). The levels of serum TNF-α and IL-33 in active RA group and the RA group in remission showed a linear positive correlation(P〈0.05). The levels of serum IL-33 were positively correlated with CCP and RF (P〈0.05). In the active RAgroup, the levels of serum TNF-α were positively correlated with DAS 28 and ESR(P〈0.05). Conclusion: IL-33 and TNF-α play important roles in the pathogenesis of RA, their relationship with the occurrence and development of RA closely, they are not only involved in the progress of RA patients, but Showed positive correlation with the activity of the diseases, especially, the expression levels of TNF-α in the serum promises to be one of the evaluation indexes of the activity RA.
