摘要
目的:探讨微创经皮锁定钢板与交锁髓内钉治疗胫腓骨中下段粉碎性骨折的疗效。方法:将2012年5月至2014年5月医院收治的70例胫腓骨中下段粉碎性骨折患者随机分为治疗1组(35例)和治疗2组(35例),治疗1组行微创经皮锁定钢板术,治疗2组行交锁髓内钉治疗,比较2组患者临床疗效、手术时间、术中出血量、骨折愈合时间、完全负重时间及并发症发生率。结果:治疗1组手术时间显著低于治疗2组、术中出血量显著高于治疗2组(P<0.05),2组间骨折愈合时间、完全负重时间比较差异无统计学意义(P>0.05);治疗1组优良率为97.1%,治疗2组为91.4%,2组优良率比较差异无统计学意义(P>0.05);治疗1组畸形愈合、关节疼痛发生率显著低于治疗2组(P<0.05),2组患者其余并发症比较差异无统计学意义(P>0.05)。结论:经皮锁定钢板术和交锁髓内钉术均是治疗胫腓骨中下段粉碎性骨折的有效方法,但是交锁髓内钉所引起的畸形愈合和关节疼痛发生率更高,经皮锁定钢板术的综合治疗效果更佳。
Objective To explore the clinical effects of interdermic fixation plate and interlocking intramedullary nail on the distal tibial and fibula comminuted fracture patients.Methods A total of 70 cases of distal tibial comminuted fracture patients were randomly and equally divided into two groups.The patients in No.1 group underwent the treatment by interdermic fixation plate,and the ones in No.2 group went through the treatment by interlocking intramedullary nail.The two groups were compared for the clinical efficacy,operating time,intraoperative blood loss,fracture healing time,full weight bearing time and complications incidence rate.Results No.1 group had the operating time significantly lower than that of No.2 group,and intraoperative blood loss statistically more than that of No.2 group(P〈0.05);there were no statistical differences between the fracture healing time and full weight bearing time of the two groups(P〈0.05).There was no significant difference between the rates for eutherapeutic response in the two groups(P〈0.05),with the rate being 97.1%in No.l group and being 91.4%in No.2 group.No.l group had the incidence rates of malunion and arthralgia significantly lower than those of No.2 group(P〈0.05),while there were no statistical differences between the incidence rates of other complications in the two groups(P〈0.05).Conclusion Both interdermic fixation plate and interlocking intramedullary nail behave well in the treatment of distal tibial and fibula comminuted fracture,while interdermic fixation plate has better clinical effect because interlocking intramedullary nail results in higher incidence rates of malunion and arthralgia.
