摘要
目的利用超声心动图观察健康成年人心脏室间隔与升主动脉长轴夹角(AoSA)及主动脉窦宽、升主动脉宽、室间隔基底段厚等参数,分析各参数与年龄、性别、体质量指数、体表面积等生理因素的关系。方法选择年龄大于18岁的健康人138例,计算体质量指数及体表面积。用超声心动图测量左室射血分数、主动脉窦宽、升主动脉宽、室间隔基底段厚度,利用中线-平行法测量AoSA。分析各参数与年龄、性别、体质量指数及体表面积的关系。结果随年龄增大,升主动脉增宽(P<0.01),AoSA减小(P<0.01)。随年龄增大,LVEF、主动脉窦部宽度均有增大的趋势。AoSA与年龄呈负相关,升主动脉、主动脉窦、LVEF与年龄呈线性正相关。各年龄组中男性主动脉窦、升主动脉较女性增宽,室间隔基底段较女性增厚,而AoSA在不同性别间差异无明显统计学意义。多因素分析显示,年龄是AoSA、LVEF、升主动脉宽度的主要影响因素;体质量指数是决定室间隔基底段厚度的主要因素,体质量指数越大,室间隔越厚。体表面积主要影响主动脉窦部宽度,对升主动脉也有一定的影响。结论随着年龄增大,AoSA减小,主动脉窦部、升主动脉增宽。随体质量指数增大室间隔厚度有增厚的趋势。临床工作中应综合考虑年龄、性别、体表面积及体质量指数对超声心动图测量指标的影响,以区别于病理改变。
Objective To observe the changes of aorta left-ventricular septal angle（AoSA）,ascending aorta（AAO）,aorta sinus,left ventricular ejective fraction（LVEF）,interventricular septal thickness at diastole（IVSd）in healthy adult by echocardiography,and to investigate their relationships with age,sex,body mass index（BMI）and body surface area（BSA）. Methods Echocardiography was performed on 138 healthy intellectuals. BMI and BSA were calculated. The LVEF,aorta sinus,AAO,IVSd were measured by echocardiography. AoSA was measured by parallel-axis way. Results With the increase of age,AoSA was decreased（P 0. 01）,AAO was increased significantly（P 0. 01）,and LVEF and aorta sinus were also increased. There was negative correlation between AoSA and age. AAo,aorta sinus and EF were positively correlated with age. There was no significant difference between male and female in AoSA. The values of AAO,IVSd were higher in male than in female. Multiple factors stepwise regression analysis showed that age was the main influencing factor of AoSA,LVEF and AAO. BSA showed the greatest effect on aorta sinus. The factor that mainly influenced IVSd was BMI. Conclusion With the increase of age,AoSA is decreased,while AAO and aorta sinus are increased. IVSd tends to thicken with BMI increasing. So it is important to comprehensively take account for the effects of age,sex,BSA and BMI on measurements by echocardiography in the clinical practice for differentiating from the pathological changes.
