摘要
目的:评价EnSite NavX三维标测系统引导下应用导管射频消融治疗房室折返性心动过速(atrioventricular reentrant tachycardia,AVRT)的效率及优势。方法:选择2012年3月至2015年6月经食管调搏提示逆行P’波呈偏心性传导的拟诊AVRT患者91例。将入选患者随机分为研究组和常规组,研究组使用EnSite NavX三维标测系统(n=51),常规组使用常规X线曝光(n=40),两组进行心内电生理检查及消融,比较两组的手术操作时间、X线曝光时间、放电次数、放电时间、手术成功率。结果:与常规组相比,研究组手术时间显著缩短[(76.7±21.3)min对(93.2±28.5)min]、X线曝光时间显著缩短[(10.2±6.7)min对(17.3±8.3)min]、放电次数显著减少[(7±3)次对(15±8)次]、放电时间显著缩短[(6.7±2.3)min对(14.2±6.7)min],P<0.01。两组手术即时成功率无统计学差异。结论:EnSite NavX三维标测系统引导射频消融治疗AVRT可明显缩短X线曝光时间与手术时间,减少放电次数和时间。
Objective:To evaluate the efficiency and advantage of radiofrequency catheter ablation(RFCA)of atrioventricular reentrant tachycardia(AVRT)under the guidance of EnSite NavX threedimensional mapping system. Methods:A number of 91 patients with AVRT received RFCA were divided into study group using the EnSite NavX system(n=51)and conventional group using X-ray(n=40).The operation time,fluoroscopy time,discharge frequency,discharge time,and success rate of operation were compared between two groups. Results:Compared with conventional group,the study group had a significantly shorter operation time [(76.7±21.3)min vs.(93.2±28.5)min],fluoroscopy time[(10.2±6.7)min vs.(17.3±8.3)min],discharge time [(6.7±2.3)min vs.(14.2±6.7)min],and significantly fewer discharge frequency [(7±3)vs.(15±8)],P〈0.01.The immediate success rate of operation had no significant difference between two groups. Conclusion:RFCA of AVRT under the guidance of EnSite NavX three-dimensional mapping system can reduce X-ray fluoroscopy time,operation time,discharge frequency and time.
