摘要
目的探讨磁共振T2mapping成像评估膝关节软骨损伤与修复的效果。方法对64例早期膝关节骨关节炎(OA)患者(OA组)及40例健康志愿者(正常组)采用磁共振T2mapping技术观察膝关节软骨的损伤与修复情况。选择16只小型雄性猪(实验组及对照组各8只)制作软骨缺损修复模型,实验组采用Mosaicplasty技术联合组织工程软骨移植进行缺损修复,对照组单纯采用Mosaicplasty技术进行移植。术后采用3T磁共振进行评估,并根据MOCART评分标准对组织修复情况进行评分。结果 OA组膝关节软骨T2值明显高于正常组、正常组和OA组负重区软骨的T2值均高于非负重区(均P〈0.05)。正常组的深层股骨髁软骨T2值均低于浅层股骨髁软骨T2值,OA组非负重区的股骨内、外髁软骨中浅层T2值均高于深层T2值(均P〈0.05);负重区股骨内、外髁软骨深及浅层T2值比较差异无统计学意义(P〉0.05)。术后18周对照组猪MOCART评分为(52.14±3.69)分,实验组MOCART评分为(63.64±7.38)分,2组比较差异有统计学意义(P〈0.05)。结论采用磁共振T2-mapping成像能有效地对无明显形态变化的软骨组织成分进行评估,对监测和诊断早期关节软组织有重要的临床价值。磁共振T2-mapping成像能有效地评估组织修复的信息,并能鉴别组织的修复情况。
Objective To evaluate knee joint cartilage damage and repair with magnetic resonance imaging T2 mapping.Methods Magnetic resonance imaging T2 mapping was performed in64 patients with early knee joint osteoarthritis(OA group)and 40 healthy volunteers(normal group)to observe knee joint cartilage damage and repair.In addition,cartilage defect was induced in 16 youth miniature pigs.These pigs were treated with mosaicplasty alone(control group,n=8)or in combination with cartilage transplantation(experimental group,n=8)for defect repair.The3 T magnetic resonance imaging was performed after operation,and the repair was assessed according to MOCART score.Results Compared with normal group,knee joint cartilage T2 values significantly increased in OA group(P〈0.05).Furthermore,compared with non-weight-bearing area,T2 values at weight-bearing area significantly increased in both groups(P〈0.05).In normal group,T2 values in superficial layer of femoral condyle cartilage were greater than those in deep layer of femoral condyle cartilage(P〈0.05).In OA group,T2 values in superficial layer of internal and external femoral condyle cartilage were greater than those in deep layer of femoral condyle cartilage at non-weight-bearing area(P 0.05),but no significant differences were found at weight-bearing area(P〈0.05).At 18 weeks after operation,MOCART score in experimental group(63.64±7.38)was significantly higher than that in control group(52.14±3.69)(P〈0.05).Conclusion Magnetic resonance imaging T2 mapping is effective for evaluating and identifying cartilage composition and repair,and has important clinical value in monitoring and diagnosing early cartilage defect.
