摘要
目的以关节镜为金标准,评价经过改良的脂肪抑制三维真实稳态自由进动序列(FS-3D-TrueFISP)对于膝关节软骨缺损性病变的诊断价值。方法采用经过改良的FS-3D-TrueFISP序列及脂肪抑制三维快速小角度激发序列(FS-3D-FLASH)对21例膝关节病变患者进行检查,其中13例患者同时行脂肪抑制快速自旋回波质子密度加权序列(FS-FSE-PDWI)扫描,将各序列诊断结果与关节镜分级诊断结果相对照。结果与关节镜结果相对照FS-3D-TrueFISP序列对于软骨缺损行病变的敏感性、特异度及表示准确度的Kappa值分别为83.3%、99.2%、0.77;FS-3D-FLASH序列相应各值分别为80.5%、97.7%、0.68;FS-FSE-PDWI序列分别为57.1%、97.6%、0.60。结论经过合理改良后的FS-3D-TrueFISP序列对于膝关节软骨缺损性病变具有较高的敏感性,应用这一技术有助于膝关节软骨缺损性病变的诊断。
Objective To evaluate the value of improved fat-suppressed-3D-true fast imaging with steady-state precession (FS-3D-TrueFISP) sequence for detecting hyaline cartilage defects of knee joint, using arthroscopy as the gold standard. Methods Twenty-one patients were imaged with fat-suppressed-3D-fast low angle shot (FS-3D-FLASH) sequence and FS- 3D-TrueFISP sequence with improved imaging parameters, and 13 patients imaging with fat-suppressed-fast spin echo-proton density weight image (FS-FSE-PDWI) sequence at same time, then compared the imaging findings with arthroscopy. Results Comparing with arthroscopy, the sensitivity, specificity and Kappa were 83.3%, 99.2% and 0. 77 respectively with FS- 3D-TrueFISP; 80. 5%, 97.7% and 0.68 respectively with FS-3D-FLASH sequence; 57.1%, 97.6% and 0. 60 respectively with FS-FSE-PDWI sequence. Conclusion The improved FS-3D-TrueFISP sequence has higher sensitivity for detecting hyaline cartilage defects in knee joint. Use of this technique may strengthen the role of MR imaging for detecting hyaline cartilage defects.
