摘要
目的:探讨规范化的家庭康复管理模式对老年脑卒中后患者吞咽功能的疗效。方法:选择2013年1月至2014年5月于我院接受治疗并出院的80例老年脑卒中后吞咽困难患者,根据随机数字表法,患者被均分为对照组(接受常规康复管理)和观察组(采用规范化的家庭康复管理模式进行干预),康复干预时间为4周。比较两组患者干预前后的吞咽功能分级及吞咽器官各指标分级的变化。结果:与干预前比较,干预后两组吞咽功能分级均有显著改善(P均<0.05),且与对照组比较,观察组1级患者比例(27.5%比52.5%)显著升高,5级患者比例(10.0%比0%)显著降低,P均<0.05。与干预前比较,干预后两组吞咽器官各指标分级均显著降低(P均<0.05),且与对照组比较,观察组舌肌[(1.60±0.24)级比(1.01±0.18)级]、咽喉肌[(1.75±0.17)级比(1.01±0.16)级]和咀嚼肌及颊肌[(1.72±0.20)级比(1.07±0.21)级]分级降低更显著,P均<0.05。结论:采用规范化的家庭康复管理模式,有利于脑卒中后患者吞咽功能及吞咽器官的功能恢复,临床效果显著。
Objective： To explore therapeutic effect of standardized family rehabilitation management mode on deglutition function in aged patients after stroke . Methods： A total of 80 aged patients with dysphagia after stroke,who were treated and discharged in our hospital from Jan 2013 to May 2014,were selected . According to random number table,patients were randomly and equally divided into control group （received routine rehabilitation management） and observation group （received standardized family rehabilitation management） , the rehabilitative intervention period was four weeks. Function classes of deglutition and deglutition organs were comparedfore and after intervention. Results： Compared with before intervention,after intervention,there were significantimprovements in deglutition function class in both groups, P 〈 0 . 05 all; compared with control group, there was significant rise in percentage of class 1 patients （27. 5% vs. 52. 5%）,and significant reduction in percentage of class 5 patients （10. 0% vs. 0%） in observation group,P〈C0. 05 both. Compared with before intervention,after intervention, there were significant reductions in all classes of deglutition organs iwithcontrolgroup,thereweresignificant reductions inclassesof lingualis [ 1.60±0. 24） classesvs. （1.01±0. 18） classes],throat muscles [ 1. 75±0. 17） classesvs. （L 01±0. 16） classes] and masticatory muscles and buccinators muscles [1. 72±0. 20） classesvs. 1.07±0. 21） classes] inobservationgroup,P〈0. 05 all. Conclusion： Standardize family rehabilitation management s helpful for function recovery of deglutition and deglutition organs and po^se^sessignificant therapeutic effects.
