摘要
目的用无创血流动力学监测系统评价心脉隆注射液治疗慢性充血性心力衰竭疗效。方法选取慢性充血性心力衰竭病患者100例,随机分成两组,每组各50例患者,对照组给予基础西药治疗,治疗组在基础治疗加用心脉隆注射液4 ml静脉点滴,2次/日,疗程5 d。观察两组治疗前后的主要症状,临床表现,用CSM 3000(香港千帆医疗仪器有限公司)无创血流动力学监测系统进行持续监测,记录两组给药前及用药5 d后的血流动力学参数,并选取其中7种:收缩压(SBP)、舒张压(DBP)、心率(HR)、心排血量(CO)、心脏指数(CI)、每搏输出量(SV)、收缩时间比率(STR)做血流动力学定量评估。结果治疗组及对照组治疗后SBP、DBP、HR与治疗前相比均有下降,而CO、CI、SV、STR与治疗前比较均有升高,差异具有统计学意义(P<0.05),且加用心脉隆注射液治疗组CO、CI、SV、STR升高较对照组明显,疗效更好,差异具有统计学意义(P<0.05)。结论心脉隆注射液能提高慢性心力衰竭的治疗效果,CSM 3000无创血流动力学监测系统在临床中应用便捷,可重复操作,能连续有效的检测观察病情变化,及时反映慢性充血性心力衰竭患者治疗过程中的病情变化,能有效的评估治疗效果,具有较强的实用价值。
Objective To evaluate the curative effect of Xinmailong Injection（XI） in the treatment of chronic heart failure（CHF） with noninvasive hemodynamic monitoring. Methods 100 cases of CHF patients were randomly divided into treatment group and control group by randomized method, 50 cases in each group. Patients in the control group received routine therapy, treatment group received routine therapy combined with XI（4ml, twice a day, the treatment lasted 5 d）. Before and after treatment, to observe the clinical symptoms by using CSM 3000（Hongkong Qianfan Medical Instrument Co. Ltd.） noninvasive hemodynamic monitoring systems for continuous monitoring. The hemodynamic parameters of two groups were recorded before and 5 d after administration. To observe the systolic blood pressure（SBP）, diastolic blood pressure（DBP）, heart rate（HR）, cardiac output（CO）, cardiac index（CI）, stroke volume（SV）, systolic time ratio（STR） before and afte treatment. Results Compared with before treatment, the level of SBP, DBP, HR of two groups after treatment decreased, the level of CO, CI, SV, STR of Two groups after treatment increased, The difference was statistically significant（P〈0.05）, and compared with control group, the level of CO, CI, SV, STR of treatment group decreased more obviously, the difference was statistically significant（P〈0.05）. Conclusion Xinmailong injection in the treatment of chronic heart failure can significantly improve the effect, CSM 3000 noninvasive hemodynamic monitoring system for clinical use is convenient, and which reflect the condition change of the process in the treatfment for patients with chronic heart failure.
