摘要
目的:应用无创血流动力学监测观察重组人脑利钠肽对顽固性心力衰竭患者血流动力学参数的影响。方法:选取2012年2月-2013年6月收治的顽固性心力衰竭患者36例,随机分为试验组(重组人脑利钠肽)和对照组。两组治疗前、后分别应用BioZ ICG无创血流动力学监测仪监测血流动力学变化。选取10种血流动力学参数:心率(HR)、收缩压(SBP)、心输出量(CO)、心脏指数(CI)、每搏输出量(SV)、每搏指数(SI)、速度指数(VI)、加速指数(ACI)、预射血期(PEP)、收缩时间比率(STR)对血流动力学进行定量分析。结果:试验组与对照组比较,应用重组人脑利钠肽治疗后,顽固性心力衰竭患者心率减慢,心输出量、心脏指数、每搏输出量、每搏指数明显提高(P<0.05),预射血期、收缩时间比率较对照组明显缩短(P<0.05);血浆脑利钠肽浓度(BNP)显著下降(P<0.05)。收缩压变化、速度指数、加速指数改善均无统计学意义。结论:重组人脑利钠肽治疗顽固性心力衰竭有着较好的血流动力学效应,而无创血流动力学监测可作为评估心衰治疗的量化指标。
Objective:To observe the effect of recombinant human brain natriuretic peptide on hemodynamic parameters in the patients with refractory heart failure.By using the noninvasive hemodynamic monitor.Methods:Thirty-six patients with refractory heart failure were randomly divided into experimental group(recombinant human brain natriuretic peptide)and control group.The BioZ ICG noninvasive hemodynamic monitoring was used before and after the treatment.To evaluate in quantification by ten hemodynamic parameters including heart rate(HR),systolic blood pressure(SBP),cardiac output(CO),cardiac index(CI)output,stroke volume(SV),stroke index(SI),velocity index(VI),acceleration index(ACI),pre-ejection period(PEP),systolic time ratio(STR).Results:After the treatment in patients with refractory heart failure with recombinant human brain natriuretic peptide,experimental group compared with control group,HR,PEP,STR in experimental group were significantly decreased(P〈0.05);CO,CI,SV and SI enxperimental group were significantly improved(P〈0.05);plasma brain natriuretic peptide concentration decreased significantly(P〈0.05).VI and ACI in experimental group were also improved,but there was no statistically significant.Conclusion:The recombinant human brain natriuretic peptide results in effective hemodynamic improvements in the patients with refractory heart failure,and noninvasive hemodynamic monitoring can be used as a quantitative index of evaluating heart failure treatment.
