双源CT Flash扫描模式对复杂性先天性心脏病的图像质量及辐射剂量研究 被引量:3

摘要 目的探讨双源CT Flash扫描模式应用于复杂性先天性心脏病的图像质量及辐射剂量。方法选择37例复杂性先天性心脏病患儿,随机分为两组,A组(20例)采用双源CT前瞻性心电门控大螺距(Flash)扫描模式,B组(17例)采用前瞻性心电门控序列(Sequence)扫描模式。两组进行双盲主观评价,再采用噪声、信噪比(SNR)、对比噪声比(CNR)等指标对图像质量进行客观评价。采用容积CT剂量指数(CTDIvol)、剂量长度乘积(DLP)、有效辐射剂量(ED)等指标评价两组辐射剂量。结果 A组图像质量主观评分为(4.25±0.89)分,高于B组(3.94±1.06)分,差异有统计学意义(P<0.05);两组客观图像质量评价差异无统计学意义(P>0.05)。两组大血管层面和心腔层面2个部分的左右心房、心室及肺动脉主干、升降主动脉ROI区的噪声、信噪比(SNR)和对比噪声比(CNR)差异均无统计学意义(P>0.05)。A组患儿接受的辐射剂量低于B组,且差异有统计学意义(P<0.05)。结论双源CT Flash扫描模式图像质量能满足复杂性先天性心脏病诊断需求,并能有效降低辐射剂量,是值得推广的扫描技术。 Objective To explore the application of the Flash spiral scanning mode of dual-source CT in the evaluation of complex congenital heart disease,and to compare with the prospective ECG-gated sequence scanning mode in image quality and radiation dose.Methods Retrospective analysis of the 37 children with complex congenital heart disease,all the cases involved were divided into group A and group B.Group A(20cases)used Flash spiral mode,and group B(17cases)used the prospective ECG-gated sequence mode.The objective evaluation of image quality were described by the noise,the signal-to-noise ratio(SNR)and the contrast-to-noise ratio(CNR).Comparison was performed between two groups in volume CT dose index(CTDIvol),dose length product(DLP),effective radiation dose(ED).Results There was statistically significant difference between the two groups,group A were significantly lower than that in group B(P〈0.05).Image quality evaluation:there were significant differences between the two groups in terms of subjective image quality assessment(P〈0.05),objective evaluation of image quality showed that although the group A is higher than group B,there was no statistical difference between the two groups of no statistical difference between the two groups(P〈0.05).Conclusions The Flash spiral mode of Dualsource CT,which was used in the diagnosis of complex congenital heart disease,can reduce the radiation dose under the condition,and could meet the requirement of the diagnosis in the image quality.This scanning mode is worth popularizing.
