摘要
目的观察比较掌骨钢板和空心钉内固定治疗后踝骨折的临床效果及安全性。方法 2010年1月—2013年12月在上海市奉贤区中心医院就诊的后踝骨折100例,依据手术方式不同将其分为观察组(53例)和对照组(47例)两组,观察组予掌骨钢板内固定治疗,对照组予空心钉内固定治疗,观察比较两组临床指标(包括术中出血量、手术时间、住院时间、骨折愈合时间及完全负重时间)、治疗24周后效果和并发症发生情况。结果治疗24周后,观察组治疗优良率为98.11%,对照组治疗优良率为80.85%,观察组治疗优良率明显高于对照组,差异具有统计学意义(P<0.05)。观察组术中出血量及手术时间较对照组明显增多或延长、住院时间及骨折愈合时间较对照组明显缩短,差异均具有统计学意义(P<0.01或P<0.05)。观察组并发症发生率为3.77%,对照组并发症发生率为19.15%,观察组并发症发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论掌骨钢板内固定治疗后踝骨折较空心钉内固定治疗后踝骨折效果确切,且并发症少,有助于踝关节早期功能锻炼,利于踝关节功能早期恢复。
Objective To compare the efficacy and their safety of metacarpal plates and hollow nail fixation in patients with posterior malleolus fracture. Methods 100 patients with posterior malleolus fracture, treated in Shanghai Fengxian District Central Hospital between January 2010 and December 2013, were divided into observation group （53 cases） and control group （47 cases） according to the different surgical procedures. The observation group was given metacarpal plate fixation and the control group was given the cannuiated screw fixation. The clinical parameter makers （including blood loss, operative time, hospital stay, fracture healing time, full weight bearing time）, efficacy of treatment after 24 weeks and their complication rates were observed in each group. Results After treatment of 24 weeks, the excellent and satisfactory rate was 98. 11% in observation group, significantly superior to that of 80.85% in control group and the difference was statistically significant （ P 〈 0. 05 ）. The blood loss and operative time were significantly more in observation group than those in control group, while the hospital stay and fracture healing time were significantly shorter in observation group than that in control group （P 〈 0.01 or P 〈 0. 05）. The complication rates were 3.77% in observation group and 19.15% in control group, significantly lower and the difference was statistically significant （P 〈 0.05）. Conclusion The efficacy of posterior malleolus fracture is more definite than that of the hollow nails with a lower rate of complications, which is helpful in early ankle exercise and quick recovery of ankle joint function.
