潮气呼吸肺功能对判断肺炎支原体肺炎婴幼儿预后的临床意义 被引量:4

摘要 目的探讨潮气呼吸肺功能测定在判断婴幼儿支原体肺炎预后中的临床意义。方法采用前瞻性的研究方法,选择2010年5月至2012年4月我院儿科病房确诊肺炎支原体肺炎的婴幼儿住院患者为研究对象(实验组),检测其急性期和缓解期的潮气呼吸肺功能:呼吸频率(RR)、潮气呼气峰流速(PEF)、每干克体重潮气量(VT/kg)、吸呼比(Ti/Te)、达峰时间比(TPTEF/TE)以及达峰容积比(VPEF/VE),并分别与同期健康婴幼儿(对照组)的潮气呼吸肺功能参数进行比较。结果实验组急性期和缓解期RR和PEF均高于对照组;而VT/kg、Ti/Te、TPTEF/TE以及VPEF/VE均低于对照组,组间比较差异均有统计学意义(P〈0.05)。两两比较发现,实验组急性期与缓解期、实验组急性期与对照组在RR、PEF、VT/kg、Ti/Te、TPTEF/TE以及VPEF/VE方面比较差异均有统计学意义(P〈0.05);实验组缓解期与对照组在PEF、TPTEF/TE和VPEF/VE方面比较差异均有统计学意义(P〈0.05),但在RR、VT/kg以及Ti/Te方面比较差异均无统计学意义(P〉0.05)。结论潮气呼吸肺功能的RR、VT/kg与Ti/Te指标可协助判断肺炎支原体肺炎婴幼儿的预后,值得临床广泛推广应用。 Objective To investigate clinical significance of tidal breath pulmonary function test in judging prognosis of mycoplasma pneumoniae pneumonia infants. Methods Infants diagnosed as mycoplasma pneumoniae pneumonia in department of pediatrics of our hospital from May 2010 to April 2012 were selected as the object of study (experimental group). Detected their tidal breath pulmonary function in acute stage and remission stage, including respiration rate (RR), peak expiratory frequency (PEF), tidal volume per kilogram (VT/kg), inspiratory time/expiratory time(Ti/Te), up to the peak time ratio (TPTEF/TE), and volume ratio of peak to peak (VPEF/VE), compared to those indexes of healthy infants (control group). Results RR and PEF value of experimental group in acute stage and remission stage were higher than those of control group; while VT/ kg, Ti/Te, TPTEF/TE, and VPEF/VE value were lower than those of control group, with statistically significant differences between two groups (P〈0.05). There were statistically significant differences in RR, PEF, VT/kg, Ti/Te, TPTEF/TE, VPEF/VE between acute stage and remission stage of experimental group, acute stage of experimental group and control group (P〈0.05); there were statistically significant differences in PEF, TPTEF/ TE, and VPEF/VE between remission stage of experimental group and control group (P〈0.05). Conclusions The indicators of RR, VT/kg, and Ti/Te in tidal breath pulmonary function can assist in judging the prognosis of infants with mycoplasma pneumoniae pneumonia, worthy of extensively clinical application.
