摘要
目的探讨BNP与cTnI在小儿心衰诊断及预后评估中的价值。方法选择2010年1月-2015年10月我院收治的心力衰竭患儿139例为观察组,选择同期于我院体检的健康小儿140例为对照组。测定两组血BNP、CTn I、CK-MB水平,检测左室射血分数(LVEF)。结果观察组BNP、cTnI及CK-MB水平高于对照组,LVEF低于对照组,差异均有统计学意义(P〈0.05);BNP诊断小儿心衰的敏感性为72.7%,特异性为82.9%,cTnI诊断小儿心衰的敏感性为37.4%,特异性为87.1%,两者联合检测诊断小儿心衰的敏感性为87.8%,特异性为92.9%;139例心力衰竭患儿经治疗出院后随访6个月,BNP水平大于500ng/L、100-500ng/L、小于100ng/L患儿其心脏事件发生率分别为60.6%、20.0%、0,差异均有统计学意义(P〈0.05)。结论联合检测BNP和cTnI对诊断小儿心力衰竭具有重要的临床意义,BNP可作为判断预后的一个参考指标。
Objective To explore the value of the level of Brain natriuretic peptide and Cardiac troponin I on diagnosis and prognosis assessment in pediatric heart failure. Methods 139 cases of pediatric heart failure from January, 2010 to October, 2015 in our hospital were selected as observation group, and 140 cases of healthy children during the same period were selected as control group. BNP, cTnI and CK-MB levels of blood plasma, and left ventricular ejection fraction(LVEF) of the two groups were detected. Results BNP, cTnI and CK-MB levels of blood plasma in observation group were higher than the control group, LVEF in observation group was lower than the control group, and the difference had statistical significance(P〈0.05). The sensitivity of BNP on the diagnosis of pediatric heart failure was 72.7%, the specificity was 82.9%. The sensitivity of cTnI was 37.4%, the specificity was 87.1%. The combined detection sensitivity was 87.8%, the specificity 92.9%. After treatment, 139 children with heart failure were followed up for 6 months after hospital discharge, the incidence of cardiac events of groups with the BNP level greater than 500 ng/L, 100-500 ng/L, less than 100 ng/L were 60.6%, 20.0%, 0 respectively, and all the differences were statistically significant(P〈0.05). Conclusion Combined detection of BNP and cTnI has important clinical significance on diagnosis of pediatric heart failure, and BNP can be served as a reference index to assess prognosis.
