摘要
目的观察308nm准分子光联合白癜风丸治疗泛发型白癜风的量效关系,并评估其安全性。方法泛发型白癜风患者60例,皮损共计147片,随机分为3组,每组各20例,A组采用308nm准分子光联合口服白癜风丸治疗,照射频率1周1次;B组采用308nm准分子光联合口服白癜风丸治疗,照射频率1周2次;C组采用308nm准分子光联合口服白癜风丸治疗,照射频率1周3次。初始以测定的最小红斑量(MED)进行局部照射,每次照射剂量递增20%~30%左右,治疗时间6个月,分别于治疗1、3、6个月后比较3组的疗效。结果治疗1个月后,B组有效率高于A组(χ2=4.408,P〈0.05),C组有效率高于B组(χ2=4.654,P〈0.05);治疗3个月后,B组有效率、显效率高于A组(χ2=6.670、4.203,P〈0.05),B、C两组有效率、显效率比较,差异无统计学意义(χ2=0.313、0.464,P〉0.05);治疗6个月后,组间比较无统计学差异(P〉0.05)。结论 308nm准分子光联合口服白癜风丸治疗泛发型白癜风,治疗初期1个月内疗效与照射频率呈相关,而1~3个月之后,疗效与照射频率无明显相关性。照射频率增加后,不良反应发生率增高,故可在治疗初期采用每周2次的照射频率,3个月后可将照射频率减至每周1次。
Objective To observe the dose effect and safety of 308 nm excimer laser combined with Vitiligo Pills in the treatment of generalized vitiligo.Methods 147 patches of lesions from 60 patients with vitiligo.These patches were divided into three groups. Group A. target lesions were treated by 308 nm excimer once a week combined with Vitiligo Pills.Group B:target lesions were treated by 308 nm excimer twice a week combined with Vitiligo Pills. Group C:target lesions were treated by 308 nm excimer three times a week combined with Vitiligo Pills.The initial dose was valued by MED,and the dose escalation of 20%-30%,the number of times 10 -30.Results After 1 month of treatment, the effective rate of group B was higher than that of group A (x^2 =4.037,P〈0.05) ,and group C was higher than that of group B (x^2 =4.288,P〈0.05). After 3 months of treatment,the marked improvement rate of group B was higher than that of group A (x^2 =5.392,P〈0.05).The effective rate between group A and B had no statistically sig- nificant difference (x^2= 3.628, P〉0.05).The effective rate and marked improvement rate be- tween group B and C had no statistically significant difference (x^2= 1.922,1.024,P〉0.05).After 6 months of treatment,the effective rate and markedly improvement rate had no statistically significant difference (P〉0.05).Conclusion In the patients with vitiligo by 308 nm excimer lasercombined with oral treatment of vitiligo pill, 1 .month after treatment,the efficacy was positively related to irradiation frequency,while there was no obvious correlation after 3 months later.The adverse reaction rate was higher when the irradiation frequency was higher, so for most pa- tients,2 times per week for the initial treatment,and 1 time per week after 3 months later.
