肥胖儿童维生素D缺乏与糖脂代谢、胰岛素抵抗的关系研究 被引量:18

摘要 目的:研究肥胖儿童维生素D缺乏与糖脂代谢、胰岛素抵抗的关系。方法:选择肥胖儿童40例、超重儿童40例、健康体检儿童40例分别纳入肥胖组、超重组、对照组,采集血清并测定1,25二羟维生素D[1,25-(OH)2D3]、糖脂代谢指标、脂肪细胞因子含量。结果:3组受试者血清1,25-(OH)2D3含量有差异(P<0.05),趋势为:对照组>超重组>肥胖组(P<0.05);空腹胰岛素(FINS)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)含量以及HOMA-IR的趋势为:对照组<超重组<肥胖组(P<0.05),高密度脂蛋白(HDL)、脂联素(APN)、Nesfatin-1、Apelin、Omentin-1、Vaspin含量的趋势为:对照组>超重组>肥胖组(P<0.05);1,25-(OH)2D3含量与FINS、TC、TG、LDL、HOMA-IR、APN、Nesfatin-1、Apelin、Omentin-1、Vaspin呈负相关,与HDL呈正相关;肥胖和超重儿童1,25-(OH)2D3缺乏者的APN、Nesfatin-1、Apelin、Omentin-1、Vaspin含量低于1,25-(OH)2D3正常者(P<0.05)。结论:肥胖儿童血清1,25-(OH)2D3含量显著降低,1,25-(OH)2D3缺乏会通过减少脂肪细胞因子的合成和分泌来造成胰岛素抵抗,进而引起糖脂代谢紊乱。 Objective:To study the relationship of vitamin D deficiency with glucolipid metabolism and insulin resistance in obese children.Methods:Forty cases of obese children,40 cases of overweight children and 40 cases of healthy children were studied and enrolled in obese group,overweight group and control group respectively,and serum was collected to detect1,25-(OH)2D3content,glucolipid metabolism indicator and adipocytokine content.Results:Serum 1,25-(OH)2D3content of three groups were different and showed the trend of control group〉overweight group〉obese group(P〈0.05);the trend of fasting insulin(FINS),total cholesterol(TC),triglyceride(TG)and low density lipoprotein(LDL)content as well as HOMA-IR was control group〈overweight group〈obese group,and the trend of high-density lipoprotein(HDL),adiponectin(APN),Nesfatin-1,Apelin,Omentin-1and Vaspin content was control group〉overweight group〉obese group(P〈0.05);1,25-(OH)2D3content was negatively correlated with FINS,TC,TG,LDL,HOMA-IR,APN,Nesfatin-1,Apelin,Omentin-1and Vaspin,and positively correlated with HDL(P〈0.05);APN,Nesfatin-1,Apelin,Omentin-1and Vaspin content in obese and overweight children with 1,25-(OH)2D3deficiency were significantly lower than those in children with normal 1,25-(OH)2D3(P〈0.05).Conclusions:Serum 1,25-(OH)2D3 content significantly decreases in obese children,and 1,25-(OH)2D3deficiency will reduce the synthesis and secretion of adipocytokines so as to cause insulin resistance and then lead to glucolipid metabolism disorder.
