摘要
目的:研究非甾体抗炎药的超前镇痛在骨科手术中的效果。方法:按照入选标准和排除标准选取2014年1月至2014年6月收治入院行骨科手术的各类骨折患者。记录每名患者人口统计学资料,骨折类型和部位,手术和麻醉方式,镇痛治疗方案,术前和术后疼痛评分,心理评分,用药期间不良反应,患者满意度评分。结果:本研究共纳入241名行骨折手术患者,分为超前镇痛组(n=115)和对照组(n=126),超前镇痛组术前使用非甾体抗炎药(主要为帕瑞昔布、塞来昔布)进行镇痛,对照组术前不使用非甾体抗炎药镇痛,两组术前疼痛评分无显著性差异。超前镇痛组和对照组在术后6 h、12 h、48 h、72 h视觉疼痛评分(visual analogue scale,VAS)、不良反应发生率和满意度评分差别有统计学意义(P<0.05)。超前镇痛组心理评分低于对照组,不良反应发生率少于对照组,满意度较高。结论:非甾体抗炎药超前镇痛运用于骨科手术中,能够有效缓解手术切口疼痛,减轻炎症反应,减少不良反应的发生,患者满意度高。
Objective: To evaluate the preemptive analgesic effect of Non-steroidal Anti-inflammatory Drugs (NSAIDs) in patients undergoing orthopedics surgery. Methods: According to inclusion criteria and exclusion criteria, patients with fracture undergoing orthopedic surgery from January 1, 2014 to April 30, 2014 were selected. Demographic data, fracture type and location, ways of operation and anesthesia, analgesic therapy, pain score before and after surgery, psychological score, adverse reaction and satisfaction of each patient were investigated. Results: This study included 241 patients undergoing orthopedics surgery, which were classified as preemptive analgesic group (n= 115) and control group (n= 126 ). N SA1Ds (mostly were parecoxib, celebrex) were used before surgery in preemptive analgesic group. No analgesic drug was given before surgery in control group. There was no significant difference of pain score between two groups before surgery. There were significant difference between two groups(P〈0.05 )in visual analogue scale(VAS), adverse reaction, patients'satisfaction at 6, 12, 48, 72 hours. Psychological score and adverse reaction in preemptive analgesic group were less than control group. Patients' satisfaction in preemptive analgesic group was higher than control group. Conelusiun: Preemptive analgesia of NSAIDs used in orthopedic surgery can reduce postoperative pain, and attenuate inflammatory and side effects.
