摘要 目的探讨血清降钙素原(PCT)在婴幼儿重症肺炎早期诊断中的价值。方法选择肺炎婴幼儿152例,其中重症肺炎患儿63例(重症肺炎组),包括细菌性肺炎32例,非细菌性肺炎31例;轻症肺炎患儿89例(轻症肺炎组),包括细菌性肺炎47例,非细菌性肺炎42例;健康体检儿童50设对照组。观察治疗前后重症肺炎组、轻症肺炎组、对照组婴幼儿血清PCT水平,比较重、轻度肺炎组患儿细菌性肺炎与非细菌性肺炎PCT水平差异,并计算PCT检查对细菌性肺炎诊断的敏感性、特异性、阳性预测值及阴性预测值。结果治疗前,3组血清PCT比较差异有统计学意义(P<0.01)。治疗后,重、中度肺炎组PCT水平均明显下降(P<0.01)。细菌性肺炎婴幼儿血清PCT水平显著高于非细菌性肺炎患儿(P<0.01)。治疗后,细菌性肺炎患儿PCT水平下降(P<0.01)。PCT检测对细菌性肺炎的敏感性、特异性、阳性预测值及阴性预测值分别为90.12%、93.22%、95.45%、85.94%。结论血清PCT检测可作为重症肺炎的客观标准,且可作为区分细菌性肺炎与非细菌性肺炎的实验室指标。 Objective To explore the value of serum procalcitonin( PCT) in the early diagnosis of children with severe pneumonia. Methods A total of 152 children with pneumonia were selected,in which 63 cases had severe pneumonia,including 32 cases of bacterial pneumonia and 31 cases of non- bacterial pneumonia. The rest 89 cases had mild pneumonia,including 47 cases of bacterial pneumonia and 42 cases of non- bacterial pneumonia. Fifty healthy children were collected as control group. The levels of serum PCT of children in severe pneumonia group,mild pneumonia group and control group were observed before and after treatment. The PCT levels in children with bacterial and non- bacterial pneumonia were compared to severe pneumonia group and mild pneumonia group,and the sensitivity,specificity,positive and negative predictive values of PCT test in the diagnosis of bacterial pneumonia were all calculated. Results There was significant difference of serum PCT level before treatment among three groups( P〈0. 01). The levels of PCT in severe pneumonia group and mild pneumonia group decreased significantly after treatment( P〈0. 01). The level of serum PCT in children with bacterial pneumonia was significantly higher than children without bacterial pneumonia( P〈0. 01),but it decreased significantly after treatment( P〈0. 01). The sensitivity,specificity,positive and negative predictive values of PCT detection to bacterial pneumonia were90. 12%,93. 22%,95. 45% and 85. 94% respectively. Conclusion Serum PCT detection can be considered as an objective standard for severe pneumonia and an experimental index for distinguishing bacterial and non- bacterial pneumonia.
