摘要
目的了解门诊医务人员手部卫生现状及影响洗手依从性的因素,探讨干预对策,提升门诊医务人员手卫生依从性。方法采用问卷设计后隐蔽式现场观察,对276名门诊医务人员的洗手方法及手卫生状况进行监测,比较不同科室、不同操作类型、不同岗位医务人员的手卫生依从率。结果影响手卫生依从性较突出原因是工作忙,没有时间洗手(29.4%),担心医患关系受影响(27.5%);不同岗位医务人员护士、医师、工勤类人员洗手依从性分别为67.7%、38.0%、29.6%;不同洗手时刻医务人员手卫生依从性最高是接触患者血液、体液后(100%),其次为接触破损皮肤、伤口敷料后(85.7%);诊疗操作后手卫生依从性(51.65%)较诊疗操作前(23.42%)高,不同科室医务人员手卫生依从率较高是普外科(71.43%)、骨科(65.63%)、检验科(52.78%)。结论全球高度关注手卫生,医院应改善管理意识,加强门诊医务人员手卫生管理,促进管理模式的形成,确保医疗安全,降低医院感染率。
Objective To analyze of the health status and the influencing factors of hand washing compliance,and to improve the hand hygiene compliance of the medical staff in clinic. Methods By using questionnaire after the hidden field observation,methods of hand washing and hand hygiene status were monitored in 276 patients.The hand hygiene compliance rate of the medical staff in different departments,different operation types and different positions were compared. Results The highlight cause of the influence of hand hygiene compliance is no time to wash their hands（ 29. 4%）; worried about relationship between doctor and patient（ 27. 5%）; the compliance of hand washing for nurses,doctors and work attendance were 67. 7%,38. 0% and 29. 6%; the highest compliance for medical staff in different hand- washing time is contact with patient blood,body fluid（ 100%）,the second is contact with damaged skin,wound dressing（ 85. 7%）; the compliance of hand hygiene after diagnosis and treatment operation（ 51. 65%） is higher than that before diagnosis and treatment（ 23. 42%）,the higher compliance of hand hygiene for medical personnel in different departments is department of general surgery（ 71. 43%）,department of orthopedics（ 65. 63%） and clinical laboratory（ 52. 78%）. Conclusion As global attention to hand hygiene,hospitals should improve management awareness and strengthen the management of hand hygiene of medical staff in clinic to promote the formation of management mode,to ensure medical safety and reduce hospital infection rate.
