摘要
目的 探讨体重指数对原发性高血压患者臂踝动脉脉搏波传导速度(baPWV)的影响.方法 收集2012年6月至2013年12月在首都医科大学附属北京安贞医院高血压科就诊的699例原发性高血压患者的病历资料,根据体重指数将患者分为体质量正常组(154例,体重指数为18.5 ~ 23.9 kg/m^2)、超重组(286例,体重指数为24.0~27.9 kg/m^2)和肥胖组(259例,体重指数为≥28.0 kg/m^2).比较3组患者baPWV水平,分析baPWV的危险因素.结果 超重组和肥胖组baPWV水平均明显高于体质量正常组[(1 650±354)、(1 682±357) cm/s比(1 602±363) cm/s],差异有统计学意义(P<0.05),超重组和肥胖组baPWV水平比较,差异无统计学意义(P>0.05).多因素Logistic回归分析发现,年龄、体重指数和舒张压是baPWV增大的独立危险因素(均P<0.05).结论 年龄、体重指数和舒张压是baPWV增大的独立危险因素.
Objective To explore the effect of body mass index (BMI) on brachial ankle pulse wave velocity (baPWV) in primary hypertension patients.Methods Intact clinical data of 699 primary hypertension patients from June 2012 to December 2013 were collected.According to BMI,all patients were divided into normal group (154 cases,18.5 kg/m^2 ≤ BMI ≤ 23.9 kg/m^2),overweight group (286 cases,24.0 kg/m^2 ≤ BMI ≤27.9 kg/m^2),obesity group (259 cases,BMI≥28.0 kg/m^2).baPWV of three groups was compared and the risk factors of baPWV was analysed.Results The levels of baPWV in overweight group and obesity group were significantly higher than those in normal group [(1 650 ± 354),(1 682 ± 357) cm/s vs (1 602 ± 363) cm/s] (P 〈0.05);there was no statistically significant difference between overweight group and obesity group (P 〉 0.05).Age,BMI and diastolic blood pressure were independent risk factors of increasing baPWV (all P 〈 0.05).Conclusion Age,BMI and diastolic blood pressure are independent risk factors of increasing baPWV.
