2型心肾综合征患者外周血IL-22和IL-27的表达及意义 被引量:2

Expression of circulating IL- 22 and IL- 27 in patients with type 2 cardiorenal syndrome and its clinical significance

摘要 目的探讨白细胞介素(IL)-22、IL-27在2型心肾综合征(cardiorenal syndrome,CRS)患者外周血中的表达。方法选择70例2型CRS患者,根据纽约心功能分级(New York Heart Function Assessment,NYHA)将入选患者分为NYHAⅡ级组、NYHAⅢ级组、NYHAⅣ级组;另选15例健康志愿者作为对照组。收集患者一般临床资料,心脏超声检查心功能变化,抽取静脉血用于检测IL-22、IL-27的表达。结果与对照组、NYHAⅡ级组相比,NYHAⅢ级组、NYHAⅣ级组患者外周血IL-22、IL-27水平显著升高(P<0.01)。外周血IL-27水平在NYHAⅣ级组患者高于NYHAⅢ级组(P<0.05),在对照组和NYHAⅡ级组间差异无统计学意义(P>0.05);在对照组与NYHAⅡ级组、NYHAⅢ级组和NYHAⅣ级组间患者外周血IL-22水平差异均无统计学意义(P>0.05)。相关性分析显示:外周血IL-22与IL-27呈正相关(r=0.75,P<0.01),IL-22与左室射血分数(LVEF)呈负相关(r=-0.68,P<0.01),IL-27与LVEF呈负相关(r=-0.64,P<0.01)。结论 IL-22和IL-27参与了2型CRS的发生、发展,这两种细胞因子可能共同促进了2型CRS的病理、生理发展。 Objective To investigate the expression of circulating IL- 22 and IL- 27 in patients with type 2 cardiorenal syndrome( CRS). Methods 70 patients with type 2 CRS were enrolled in the study. According the New York Heart Function Assessment( NYHA),patients were divided into NYHA class Ⅱ group,NYHA class Ⅲ group and NYHA class Ⅳ group. 15 healthy volunteers were enrolled as the control group. General clinical data were collected and echocardiography was performed to evaluate heart function. Venous blood was extracted to detect the expression of IL- 22 and IL- 27. Results Compared with the control group and NYHA class Ⅱ group,circulating IL- 22 and IL- 27 were increased in the NYHA class Ⅳ group and NYHA class Ⅲ group( P〈0. 01). The levels of IL- 27 were higher in the NYHA class Ⅲ group than in the NYHA class Ⅳ group( P〈0. 05),and were similar between the control group and NYHA class Ⅱ group( P〉0. 05). The levels of IL- 22 were similar among the control group,NYHA class Ⅱ group,NYHA class Ⅲ group and NYHA class Ⅳ group( P〉0. 05). Correlation analysis showed that circulating IL- 22 level was positively correlated with circulating IL- 27( r = 0. 75,P〈0. 01),and negatively correlated with LVEF( r = 0. 75,P〈0. 01). Circulating IL- 27 was negatively related with LVEF( r = 0. 64,P〈0. 01). Conclusion IL- 22 and IL- 27 is involved in the development of type 2 CRS,and may promote the pathophysiological development of type 2 CRS.
