摘要
【摘要】目的探讨心肌酶水平在新生儿黄疸患儿中的表达及临床意义。方法新生儿病理性黄疸患儿85例为观察组,选择同期生理性黄疸新生儿45例为对照组,观察组患儿发现黄疸、治疗前检测血清胆红素及心肌酶水平,对照组新生儿于入院当天、治疗前检测血清胆红素及心肌酶水平。观察组患儿采用综合对症治疗,5d后检测并比较观察组患儿治疗前后血清胆红素及心肌酶水平,不同程度血清胆红素水平新生儿心肌酶水平,并与对照组比较。结果观察组血清胆红素及心肌酶水平均显著高于对照组(P〈0.01),高胆红素水平患儿心肌酶水平明显高于低胆红素水平患儿(P〈0.05);观察组患儿治疗5d后血清胆红素及心肌酶水平均较治疗前显著下降(P〈0.01),与对照组比较差异无统计学意义(P〉0.05)。结论新生儿黄疸患儿可伴有心肌损害,损害程度与黄疸严重程度相关,心肌损害可随黄疸消退而恢复正常,应对新生儿黄疸患儿常规检测心肌酶,并加强对心肌细胞的保护。
Objective To explore the experession level and clinical significance of myocardial enzyme level in children with neonatal jaundice. Methods 85 cases of neonates with pathologic jaundice were treated as observation group, and meanwhile 45 cases of neonates with physiologic jaundice were treated as control group. The serum bilirubin and the myocardial enzyme level of two groups of neonates were both detected and compared before the treatment on the day of admission, and the myocardial enzyme level of neonates with different levels of serum bilirubin. The neonates in observation group adopted the comprehensive corresponding measures to treat, to detect and compare the serum bilirubin and the myocardial enzyme level of the neonates of the observation group before and after treatment after 5d, and to compare with the control group. Results The serum bilirubin and the myocardial enzyme level of the observation group were significantly higher than those of the control group (P〈0.01) , and the myocardial enzyme level of hyperbilirubinemia neonates was obviously higher than those of hypobilirubinemia neonates ( P〈0.05 ) . The serum bilurubin and the myocardial enzyme level of the neonates observation group after treatment of 5d was significantly lower than those before treatment (P〈0.01) , and there was no significant difference in the comparison with the control group. Conclusion Children with neonatal jaundice can be accompanied by cardiac damage, and the extent of damage is related to the severity of jaundice. Cardiac damage can be resolved and return to normal with jaundice. Children with neonatal jaundice should be routinely detected in myocardial enzyme, and the protection of myocardial cells should be strengthened.
