摘要 目的:探讨^(18)F-FDG PET/CT显像在多发性骨髓瘤诊断及分期中的临床价值。方法:回顾性分析45例经临床及病理证实的多发性骨髓瘤患者PET/CT影像资料,分析并总结PET/CT显像特征与对应DS(Durie-Salmon)plus分期。结果:42例患者出现不同程度的骨质破坏,2例全身广泛性骨质疏松,1例全身骨质无明显破坏,FDG代谢轻度增高,分析得到^(18)F-FDG PET/CT显像对多发性骨髓瘤诊断的灵敏度为86.7%、准确性为86.7%。DS plus分期:Ⅰ期6例,Ⅱ期16例,Ⅲ期23例。结论:^(18)F-FDG PET/CT显像对多发性骨髓瘤的诊断具有重要的临床应用价值,可作为肿瘤分期的有效补充手段。 Purpose:To assess the clinical value of 18F-FDG PET/CT in the diagnosis and staging of multiple myeloma(MM). Methods:The 18F-FDG PET/CT images of 45 patients who were confirmed with MM clinically and pathologically were analyzed retrospectively. The imaging characteristics of PET/CT and the staging according to Durie Salmon plus system were analyzed and summarized. Results:Of all 45 patients detected by ^18F-FDG PET/CT, 42 patients had bone destruction, 2 had systemic osteoporosis, and 1 had no bone destruction but with mild increase of ^18F-FDG uptake. The sensitivity and accuracy of PET/CT in diagnosis of MM were all 86.7%. The DS plus staging results were as followed:6 cases were staging I,16 cases were staging II, and 23 cases were staging III. Conclusion:18F-FDG PET/CT plays an important role in the diagnosis of MM, and it can be used as an effective supplementary method for tumor staging.
