64排CT与磁共振在隐匿性骨折诊断中的比较分析 被引量:8

摘要 目的探讨64排螺旋CT与磁共振对隐匿性骨折的比较诊断价值。方法收集临床确诊隐匿性骨折87例,经X线片检查未发现骨折,分别进行64排螺旋CT与磁共振检查,比较两种检查方法的诊断结果及表现。结果 MRI对隐匿性骨折的总检出率(95.56%)明显高于64排螺旋CT(78.57%),差异有统计学意义(P<0.05)。CT组单一横断面显示骨折22例(66.67%),横断结合三维重建共显示骨折11例(33.33%)。MRI组43例(95.56%)的T1WI图像显示骨皮质下有不规则的线状、条状的低信号,而T2WI图像显示为高信号。结论 64排螺旋与MRI均是诊断隐匿性骨折的有效方法,但磁共振对隐匿性骨折的检出率高于64排螺旋CT,更具临床优势。 Objective To explore the value of 64 slice spiral CT and magnetic resonance imaging( MRI) in the diagnosis of occult fracture. Methods Eighty- seven cases of patients with occult fracture which X- ray detection failed to detect. Then Magnetic resonance examination and 64 slice spiral CT examination were performed,and the results of both methods were compared. Results The total detection rate of occult fracture in MRI group( 95. 56%) was significantly higher than that of CT group( 78. 57%),with a statistically significant difference( P〈0. 05). In the CT group,22 cases( 66. 67%) were identified with fracture by transaxial image,and transaxial image combined with 3D reconstruction technology identified 11 cases( 33. 33%) with fracture. In the MRI group,T1 WI images of 43 patients( 95. 56%) showed weak irregular line or bar signal under bone cortex,and T2 WI images showed strong signal. Conclusion Both 64 slice spiral CT and MRI are effective in diagnosis of occult fracture,but the detection rate of MRI is higher than that of 64 slice spiral CT,which shows MRI is more valuable in clinical diagnosis.
