摘要
目的：评价原因不明复发性流产（URSA）主动免疫治疗的临床效果。方法：2001年2月至2015年2月在我院门诊就诊的URSA患者1005例为研究对象，取其丈夫或与女方无血缘关系的第三方男性的肘静脉血20ml，分离淋巴细胞于孕前后注射于患者前臂，使其产生主动免疫，并随访妊娠结局。结果：1005例URSA患者中共妊娠894例（88．96％），其中足月分娩718例（71．44％），随访截止时，尚在妊娠中91例（9．05％），再次自然流产85例（8．46％）；无早产病例；未受孕111例（11．04％）。年龄〉32岁者流产率明显高于≤32岁者（P〈0．05）。孕后免疫治疗2～3次者比孕后未治疗或仅治疗1次者的足月分娩率明显更高（P〈0．05），流产率明显更低（P〈0．05）。结论：主动免疫治疗URSA患者疗效肯定，妊娠成功率高，年龄大于32岁者妊娠后应早期保胎治疗；免疫治疗后3个月内受孕，确认早孕后再免疫治疗2～3次效果更好。
Objective：To evaluate the clinical effect of active immunotherapy on unexplained recurrent sponta- neous abortion（URSA）. Methods： 1005 cases of URSA form 2001 to 2015 in our hospital were taken into the study. Lymphocytes separated from the peripheral blood of their spouses were injected into their foreams to in- duce active immunization. Pregnancy outcomes were assessed. Results： Among the 1005 cases,894 cases （88. 96% ） were conceived and 718 cases（71.44% ） delivered successful termly,91 cases（9.05% ） were still in the process of pregnancy, and 85 cases （8.46%） underwent spontaneous abortion once again, and 111 cases （11.04%） were infertility. The abortion rate in the patients with age 〉 32 years was significantly higher than that in patients with age≤32 years（ P 〈0. 05）. Compare to the patients who received no or only one time immuno- therapy,the rate of full-delivered in the patients who received 2 -3 times immunotherapy was significantly higher （ P 〈 0.05）, and the rate of abortion was significantly lower （ P 〈 0.05）. Conclusions： Active immunization is effect in treatment of URSA,which can improve the pregnancy maintenance in URSA patients. Managements to support pregnancy are preferred to those whose ages excessed 32 years. At least 2 -3 time immunotherapies are recommended before and after conception.
