摘要
目的探讨肺炎支原体DNA(MP-DNA)拷贝数在儿童肺炎支原体肺炎中的价值。方法收集住院确诊肺炎支原体肺炎的患儿310例,分别进行痰肺炎支原体DNA检测及血清肺炎支原体IgM抗体(MP-IgM)检测,检测结果与临床病情进行比较。动态监测MP-IgM,收集120例支原体肺炎患儿血清IgM抗体近期由阴性转为阳性,且双份血清抗体滴度增高4倍,对这些患儿进行痰肺炎支原体DNA的检测。结果 310例患儿中DNA拷贝数103有63例,其中重症或大叶性肺炎3例,所占比例为4.76%;DNA拷贝数104有55例,其中大叶性肺炎或重症肺炎15例,所占比例为25.45%;DNA拷贝数105有57例,其中大叶性肺炎或重症肺炎17例,所占比例为29.82%;DNA拷贝数106有77例,其中大叶性或重症肺炎有44例,所占比例为57.14%;DNA拷贝数107有58例,其中大叶性或重症肺炎37例,所占比例为63.79%。120例支原体肺炎患儿双份血清由阴性转为阳性,且双份血清抗体滴度增高4倍以上者行痰MP-DNA检测,120例患儿中有103例支原体DNA拷贝数在105-107,所占比例高达85.83%。结论呼吸道痰液中MP-DNA拷贝数越高越支持肺炎支原体的感染,呼吸道痰液中MP-DNA拷贝数越高越支持重症支原体肺炎或大叶性支原体肺炎的诊断。
Objective To explore evaluation of mycoplasma pneumoniae DNA copy number detection in children with mycoplasma pneumoniae pneumonia.Methods Collect in-patients diagnosed mycoplasma pneumonia in 310 cases.TO test Mycoplasma pneumoniae DNA in sputum and serum IgM antibody of mycoplasma pneumonia,The results were compared with clinical condition.Dynamic analysis of serum IgM antibody of mycoplasma pneumonia.Collect 120 cases of mycoplasma pneumonia serum IgM antibody recently was changed from positive to negative and double antibody titer in serum was 4 times of sputum detection of Mycoplasma pneumoniae DNA of these children.Results 310 cases of the DNA copy number of103 in 63 cases,including 3 cases of severe or lobar pneumonia,accounted for 4.76%;the copy number of DNA104 in 55 cases,including 15 cases of lobar pneumonia or severe pneumonia,accounted for 25.45%;the copy number of DNA105 in 57 cases,including lobar pneumonia or severe pulmonary inflammation in17 cases,accounted for 29.82%;the copy number of DNA106 in77 cases,including lobar or severe pneumonia in 44 cases,accounted for 57.14%;the copy number of DNA107 in 58 cases,including 37 cases of lobar pneumonia or severe pneumonia,the proportion of 63.79%.120 cases of mycoplasma pneumonia in double serum from negative to positive and the increase of double serum antibody titer was 4 times higher.For the MP-DNA detection of sputum in 120 cases,103 cases of Mycoplasma DNA copy number in 105-107,the proportion is as high as 85.83%.Conclusion The higher the copy number of MP-DNA infection of Mycoplasma pneumoniae in sputum for diagnosis,the copy number of MP-DNA is higher in support of severe mycoplasma pneumonia or lobar Mycoplasma pneumonia in sputum.
