枣庄市2011—2014年布鲁氏菌病流行病学特征分析 被引量:10

Epidemiological characteristics of brucellosis in Zaozhuang from 2011 to 2014

摘要 目的了解枣庄市2011—2014年布鲁氏菌病(以下简称布病)流行病学特征,为制定防治策略提供科学依据。方法用描述流行病学方法分析中国疾病报告信息系统2011—2014年布病三间分布特征,运用excel、SPSS 17.0软件进行数据分析。结果枣庄市2011—2014年累计报告布病确诊病例166例,无死亡病例,年发病率分别为0.107 3/10万(4例)、0.532 9/10万(20例)、1.407/10万(53例)、2.350 3/10万(89例)。发病高峰在3—10月,共报告139例,占病例总数的83.73%,5月病例数最多,共报告20例,占总数的12.05%。病例最小5岁,最大75岁,60岁组病例占总数的18.07%,男女之比2.19:1,以农牧民为主,占总病例数的81.33%,职业分布差异有统计学意义(χ2=7.844,P<0.001)。2011年仅有1个区发生病例,疫区逐年扩大到全市6个市(区),发病率最高是峄城区2.22/10万(33例),最低是薛城区0.51/10万(10例)。结论枣庄市布病发病数逐年增加,波及范围逐年扩大,病例以青壮年农民为主。建议建立枣庄市布病防控联防机制,完善人畜传染源监测和防控知识的宣教工作,提高人群自我防范意识。 Objective To explore epidemiological characteristics of brucellosis in Zaozhuang City during 2011-2014 toprovide evidences for strategy of control and prevention. Methods Descriptive epidemiological method was applied toanalyze the three distribution characteristics of brucellosis from China Diseases Reporting Information System from 2011 to2014. Excel and SPSS 17.0 were used for data analysis.Results During 2011-2014, 166 cases of brucellosis were reportedwithout mortality. Annual incidence rates were 0.107 3/lakh(4 cases), 0.532 9/lakh(20 cases), 1.407/lakh(53 cases) and2.350 3/lakh(89 cases) respectively. The incidence peaked between March and October with 139 cases reported accounting for83.73% of the total cases. The largest number of cases was in May with 20 cases accounting for 12.05%. Age ranged from 5 to75 years old with 60 years old accounting for 18.07%. Significant differences were observed in age and occupation distributions(χ2=7.844, P0. 001) with a male-to-female ratio of 2. 19: 1, and farmer as the predominant population(81.33%). Epidemicsof brucellosis initiated in one county in 2011, and expanded yearly to all six counties(districts) with the highest incidence of2.22/100 000(33 cases) in Yicheng district, and the lowest incidence of 0.51/100 000(10 cases) in Xuecheng district.Conclusion The incidence of brucellosis increased year by year in Zaozhuang with outspreading. Young- and middle-agedfarmers were most susceptible for the infection.Mechanism of prevention and control for brucellosis is recommended to beestablished in Zaozhuang. Surveillance for the source of infection in human and animal should be consummated. Publicitywork for prevention and control should be promoted. Self-protection awareness should be improved.
