摘要
目的:探讨采用手机APP服务平台进行乳腺癌术后化疗患者维护的可行性,为临床实践提供依据。方法:将638例乳腺癌术后化疗患者随机分为观察组320例和对照组318例,观察组采用手机APP程序搭建术后维护平台,定期推送体检信息、关爱提醒、健康监测和健康教育等服务;对照组采用常规出院流程,包括出院带药使用方法及注意事项,交代家中护理注意事项,嘱定期复诊等。随访1年,比较两组健康知识掌握情况、化疗不良反应发生率、来院检测次数及用药依从性、心理状况和生活质量等指标。结果:观察组健康知识掌握率明显优于对照组(P<0.05,P<0.01);观察组术后化疗不良反应发生率明显低于对照组,来院检查次数和Morisky评分明显高于对照组(P<0.05,P<0.01);观察组抑郁、躯体功能、心理功能评分及总分高于对照组,焦虑评分低于对照组(P<0.05,P<0.01)。结论:基于手机APP服务构建乳腺癌术后化疗患者维护平台,可提高患者健康知识掌握率,降低不良反应发生率,提高患者遵医行为,改善患者心理状况和生活质量。
Objective： To explore the feasibility of using APP service platform of mobile phone for the postoperative nursing care of breast cancer patients with chemotherapy after discharge in order to provide the basis for clinical practice. Methods： 638 postoperative patients undergoing chemotherapy were randomly divided into the observation group（ n = 320） and the control group（ n = 318）. The APP service platform of mobile phone was adopted in the observation group to regularly provide the service like medical information,concern,health monitoring and health education; the conventional discharge guidance was given to the patients in the control group. After one year of follow- up,the patients＇ awareness of health knowledge,the incidence of adverse drug reaction,the number of reexamination,medication compliance,psychological status and quality of life of the patients were compared between the two groups. Results： The patients＇ awareness of health knowledge was significantly better in the observation group than the control group（ P〈0. 05,P〈0. 01）; the incidence of adverse drug reaction was significantly lower and the Morisky score was higher in the observation group than the control group（ P〈0. 05,P〈0. 01）; the scores of depression,somatic function and psychological function and the total score were higher and the score of anxiety was lower in the observation group than the control group（ P〈0. 05,P〈0. 01）. Conclusion： Application of APP service platform of mobile phone to postoperative nursing care of breast cancer patients with chemotherapy after discharge can improve the patients＇ awareness of health knowledge and reduce the incidence of adverse drug reactions,and it can also improve patient＇s compliance,psychological status and quality of life.
