乳腺癌危险因素的病例对照研究 被引量:14

摘要 目的:探讨女性乳腺癌的危险因素,为有效的预防控制措施的建立提供依据.方法:应用成组病例对照的研究方法,收集经组织病理学确诊为乳腺癌的女性病例121名及社区来源的健康女性对照211名.采用统一的问卷调查表获取病例和对照乳腺癌相关危险因素的信息.采用非条件logistic回归进行单因素和多因素分析.结果:多因素分析:与乳腺癌发病有关的变量为吸烟、未足月产次数多及家禽肉类摄入量多,OR值及95%CI分别为15.093(1.014~224.622)、1.759(1.006~3.073)和1.811(1.024~3.203);对乳腺癌有保护作用的变量是奶制品及豆制品摄入量多,OR值及95%CI分别为0.763(0.624~0.933)和0.626(0.422~1.044).经分组分析,绝经组奶制品摄入量多具有保护作用;非绝经组奶制品及豆制品摄入量多具有保护作用,而吸烟增加患乳腺癌的危险.结论:吸烟、未足月产次数多、家禽肉类摄入量多是乳腺癌的危险因素,奶制品及豆制品摄入量多是乳腺癌的保护因素. Objective: To study the major risk factors of female breast cancer in Sichuan, a case oontrol study was designed. Methods: 121 breast cancer patients with histopathological diagnosis and 211 healthy oommunity controls were included. All data were collected with a unified questionnaire. Unconditional logistic regression model was used to analyzed the significant risk factors. Results: Active smoking, abortion, taking poultry, milk and .soybean food frequently were related to breast cancer, the ORs were 15.093 (1.014-224.622), 1.759 (1.006-3.073), 1.811 (1.024-3.203), 0.763 (0.624-0.933) and0.626 (0.422- 1.044), respectively. Subgroup analyses showed that taking milk frequently Call decrease risk of breast cancer in postmenopausal women, while risk in pre- menopausal women were associated with milk intake, soybean food intake ,and smoking. Conclusion: Taking poultry frequently, abortion and active smoking are risk factors for breast cancer, while taking milk and soybean food frequently have beneficial effects of protecting individual from breast cancer.
