摘要
[目的]通过观察纤维乳管镜下药物灌注鱼腥草注射液或庆大霉素注射液治疗非肿瘤性乳头溢液的临床疗效,探讨可能的作用机制。[方法]收集符合纳入标准患者120例,随机分成鱼腥草灌注治疗组和庆大霉素灌注治疗组,每周治疗1次,直至溢液停止。每次治疗前观察溢液量的变化并检测溢液中TNF-α和IL-6含量(ELISA法)。[结果]鱼腥草治疗组治愈率是96.43%,庆大霉素治疗组是77.59%,鱼腥草治疗组治愈率高于庆大霉素治疗组,差异具有统计学意义(P<0.05);而且鱼腥草治疗组中位治愈时间为3周,庆大霉素治疗组中位治愈时间为5周,鱼腥草治疗组比庆大霉素治疗组治愈时间短,差异具有统计学意义(P<0.05);两治疗组治疗后较治疗前乳头溢液中IL-6和TNF-α的含量明显降低,差异具有统计学意义(P<0.05);治疗后鱼腥草治疗组乳头溢液的IL-6、TNF-α含量均低于庆大霉素治疗组,差异具有统计学意义(P<0.05)。[结论]纤维乳管镜下药物灌注治疗非肿瘤性乳头溢液总有效率鱼腥草治疗组优于庆大霉素治疗组;纤维乳管镜下药物灌注治疗非肿瘤性乳头溢液的机制可能与灌注药物能降低溢液中IL-6和TNF-α的含量有关。
[Objective]To explore the mechanism by observing the clinical effects of drug perfusion of houttuynia injection or gentamicin injection on non- neoplastic nipple discharge with Fiberoptic ductoscopy.[Methods] 120 cases were assigned randomly into houttuyniae injection group and gentamicin injection group. All patients were treated once a week, until the discharge stopped. Before every treatment, observe the change of discharge amount and TNF-alpha and IL-6 levels in discharge were evaluated by ELISA. [Results]The cure rate was 94.9% in group treated with houttuynia versus 82.8% in gentamycin group（P〈0.05） The cure time was 3.67 weeks in group treated with houttuynia versus 4.24 weeks in gentamycin group（P〈0.05）.The levels of IL-6 and TNF-alpha in nipple discharge after treatment were less than those before treatment in two groups, the difference was statistically significant（P〈 0.05）.The levels of IL-6 and TNF-alpha in nipple discharge after treatment in houttuynia group were less than those in gentamycin group（P〈0.05）. [Conclusion]For non-neoplastic nipple discharge, houttuynia injection treatment under Fiberoptic ductoscopy was superior to gentamicin injection. The mechanism of perfusion chemotherapy treatment of non-neoplastic nipple discharge with fiber laticifer microscopically may be related to the effect which the perfusion chemotherapy can reduce the discharge of IL-6 and the content of TNF-alpha.
