θ短阵快速脉冲模式重复经颅磁刺激对精神分裂症疗效及对其执行功能的影响 被引量:16

摘要 目的探讨θ短阵快速脉冲(TBS)模式重复经颅磁刺激治疗精神分裂症的疗效及对其执行功能的影响。方法将90例精神分裂症患者按照不同治疗方式分为研究组(TBS治疗)和对照组(伪刺激治疗)各45例,共治疗4周。于治疗前及治疗后第4周末应用威斯康星卡片分类测验-64(WCST-64)、阳性和阴性综合征量表(PANSS)、定步调听觉连续加法测验(PASAT)、词汇流畅性测试(VFT)评定两组执行功能及其疗效指标改善情况,采用治疗中需处理的不良反应症状量表(TESS)评定不良反应。结果治疗后第4周末研究组WCST-64持续错误数、持续反应数、完成分类数均较治疗前降低(P<0.05),并低于对照组(P<0.05)。两组治疗后第4周末PANSS、PASAT、VFT评分均较治疗前升高(P<0.05),研究组PANSS、PASAT、VFT评分高于对照组(P<0.05)。两组间总不良反应发生率比较差异无统计学意义(P>0.05)。结论 TBS模式重复经颅磁刺激可有效改善精神分裂症患者的症状和执行功能,不良反应少。 Objective To explore the effects of repetitive transcranial magnetic stimulation with Theta burst stimulation paradigm( r TMS with TBS) to improve the symptoms and executive function in patients with schizophrenia. Methods A total of90 patients with schizophrenia were divided into study group( n = 45) treated with r TMS with TBS and control group( n = 45)treated without r TMS with TBS for the treatment of 4 weeks. They were assessed with Wiscosin Card Sorting Test( WCST),Positive and Negative Syndrome Scale( PANSS),Paced Auditory Serial Addition Test( PASAT),Verbal Fluency Test( VFT) and Treatment Emergent Symptom Scale( TESS) at baseline and the 4^(th)weekend of treatment. Results The perseverative responses errors,perseverative responses and categories completed of WCST in study group at the 4^(th)weekend of treatment were all significantly less than those at the baseline( P〈0. 05). At the 4^(th)weekend of treatment,the perseverative responses errors,perseverative responses and categories completed of WCST in study group were all significantly less than those in control group( P〈0. 05). The scores of PANSS,PASAT and VFT in two groups at the 4^(th)weekend of treatment were all significantly higher than those at baseline( P〈0. 05). At the 4^(th)weekend of treatment,the scores of PANSS,PASAT and VFT of WCST in study group were all significantly higher than those in control group( P〈0. 05). There was no difference for the total rate of side effects between two groups at the 4^(th)weekend of treatment( P〉0. 05). Conclusion It's effective and safe for r TMS with TBS to improve the symptoms and executive function in patients with schizophrenia.
