经颅多普勒超声联合颈部血管超声对脑梗死的诊断价值 被引量:7

摘要 目的探讨经颅多普勒超声(TCD)联合颈部血管超声(CUS)对脑梗死的诊断价值。方法选取108例脑梗死患者为脑梗死组,将同期在该院体检的108例健康志愿者作为对照组,两组研究对象均接受TCD+CUS检查。对比两组血流异常率,并评估TCD+CUS与头颅CT+MRI诊断脑梗死的符合率。结果脑梗死组血流异常率为61.1%(66/108),明显高于对照组的12.0%(13/108),两组比较,差异有统计学意义(P<0.05);TCD联合CUS与CT+MRI诊断前循环梗死、后循环梗死及分水岭区梗死的符合率分别为81.6%(40/49)、92.0%(23/25)和77.8%(14/18)。结论 TCD联合CUS检查可作为脑梗死患者的有效辅助诊断手段,能降低误诊或漏诊风险。 Objective To explore the value of transcranial Doppler ultrasound (TCD) combined with neck vascular ultrasound (CUS) on diagnosis of cerebral infarction. MethodsA total of 108 patients with cerebral infarction were selected as cerebral infarction group; 108 healthy volunteers received physical examination in our hospital were enrolled in the control group. Both the research subjects of the two groups were received TCD+CUS examination. The rate of abnormal blood flow was compared between the two groups, and the coincidence rate of TCD+CUS and CT+MRI on diagnosis of cerebral infarction were evaluated. ResultsThe abnormal rate of blood flow in the cerebral infarction group was 61.1% (66/108), higher than that of the control group (13/108), the difference was statistically significant (P〈0.05). The coincidence rate of TCD+CUS and CT+MRI on the diagnosis of anterior circulation infarction, posterior circulation infarction and watershed infarction were 81.6% (40/49), 92% (23/25) and 77.8% (14/18),respectively. Conclusion TCD combined with CUS can be used as an effective assistant diagnostic method for patients with cerebral infarction, and reduce the risk of misdiagnosis or missed diagnosis.
